Job Title: Social Worker - Pediatric Hematology

_Social Work Services at the Mount Sinai Hospital_

The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.

+ In this full-time, multi-source-funded position, the social worker will be responsible for providing comprehensive and continuous social work services to children with sickle cell disease as well as other hematology diagnoses such as hemolytic anemias, thrombocytopenias, beta thalassemias, hemangiomas, and deep vein thrombosis.

+ Patients are followed in ambulatory care settings, on the inpatient units including the Pediatric Hematology/Oncology Unit, the Pediatric Intensive Care Unit and in the transfusion room during procedures.

+ The social worker assists children and families with the impact of the diagnosis and the implications of chronic illness requiring treatment across the lifespan.

Qualifications

+ NYS LMSW or LCSW

+ Ability to work in a high-pressure environment with children with life-threatening illnesses which includes end-of-life and bereavement work.

+ Strong organizational, collaborative, and teamwork skills.

+ Some experience working with critically/chronically ill children and end-of-life/bereavement _preferred._

+ Bilingual (English/Spanish) _preferred._

+ Internal applicants: Minimum one year in current position of experience in Women & Children’s Health.

Compensation Statement

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $85,280.00 - $97,760.36 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.

Responsibilities

+ The social worker functions as a member of a large, complex multi-disciplinary team and closely collaborates with pediatric oncology social workers, including some cross coverage.

+ The social worker will work with both the adult sickle cell team to implement transition programming and will collaborate with team to identify program development including groups for children, adolescents and parents.

+ The social worker will be responsible for supporting team-based patient care through such activities as making arrangements and referrals for patient transportation, home care, nutritional services and providing discharge planning for inpatients.

+ Skills in working with a complex, evolving team of multidisciplinary providers is critical as is an ability to work in a fast paced, demanding environment.

+ The social worker is also responsible for pediatric program wide coverage through a rotational schedule several times per month.
