    Title: Health Services Representative

    Your Specific Responsibilities:

    Working under the direct supervision of the Sexually Transmitted Disease (STD) Program Supervisor, this position provides direct disease and public health awareness intervention services to the Department of Health in Collier County (DOH-Collier) STD Program. These activities include partner counseling and referral services (PCRS), sexually transmitted infections (STI) case management, investigation and intervention by notification, counseling, and referral of exposed individuals or at-risk of STI/HIV infections. This position requires knowledge of the public health system and professional skills to interact with patients, health care providers and others in the community involved in STI/HIV prevention and control; and the ability to interact with people with diverse social, economic, and cultural lifestyles and characteristics. This position also requires communications skills to interview and elicit information from patients, contacts, and at-risk individuals. This position is at risk for exposure to blood borne and respiratory pathogens (moderate risk level) under OSHA guidelines and requires annual Tuberculosis (TB) assessment which may include a TST/ and interferon gamma release assay (IGRA) test and or chest x-ray and is required to have up-to-date vaccinations for some vaccine-preventable diseases. Contributes to the organization’s culture of performance excellence by actively seeking to improve personal and organizational performance through measurement, analysis, knowledge management, and processes improvements and participation in performance management committees, councils, and/or workgroups. Continuously demonstrates the Department’s ICARE values. The following are considered essential job specific duties for this position:

     

    Conducts investigations to locate infected individuals with STIs and refers them for medical evaluation, treatment and preventive counseling with their contacts and at-risk individuals. Ensures that all activities are conducted under the Federal, State, and local program operational procedures and guidelines. Provides PCRS in accordance with current STD program policy to a wide variety of individuals infected with or at risk of STIs/HIV.

     

    Performs case management activities directed at identifying infected individuals with STIs, work prioritization and record management, STARS data entry and HIV pre- and post- counseling sessions. Communicates with public and private clinics, clinicians, hospitals, laboratories, and other health care provider to obtain patient related information to assist in the location, treatment, and prevention of STI/HIV.

     

    Provide counseling, education, referrals, and linkage to care to newly diagnosed HIV clients and high-risk referrals to Pre exposure prophylaxis for HIV (PrEp)

     

    Performs clinical functions, obtains blood and urine specimens, prepares laboratory orders, and manifests and refers patients for medical evaluation and treatment

     

    Completes required records and reports in accordance with DOH - Collier STD program policies and procedures. Keeps record and educational materials. Secures all confidential information in the office, on computers and in the field.

     

    Participates in clinical, community or impromptu screening activities and/or community education and/or targeted outreach programs. These activities will occur before, during or after normal working hours and/or weekends or holidays.

     

    Provide STD, HIV, and Hepatitis screenings during Free Testing Events at DOH-Collier.

     

    Order and maintain up-to-date inventory of clinic and outreach supplies, and condoms.

     

    Performs other duties as assigned.

    Required Knowledge, Skills, and Abilities:

    Effective interpersonal skills in multicultural settings.

     

    Good organizational skills.

     

    Skilled in proficiency in the use of the following computer systems: Microsoft office, STARS and HMS

    Qualifications:

    Minimum

     

    Current Florida Driver’s License and reliable transportation and willingness to use personal vehicle for business related activities.

     

    Emergency Duty - Incumbent may be required to work during or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or to perform other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural (F.S. 242.42).

     

    N-95 Mask – Ability and willing to wear an N-95 respirator.

     

    Bilingual or multilingual: English/Creole (French), English/Spanish, English/French, English/Spanish/Creole (French).

     

    Experience performing venipuncture.

    Trainings:

    Introduction to STARS (or to be completed within 3 months of employment).

     

    HIV/AIDS 500/501 Counseling and Testing Certification (or to be completed within 6 months of employment)

     

    Passport to Partner Services Training (or to be completed within 6 months of employment),

     

    Preferred

     

    Bachelor’s degree in health or social sciences

     

    Experience working in an STI prevention & surveillance program.

     

    Experience working in case management.
