    Summary

    The CHW II will work closely with various CHA departments and community partners who serve as a liaison between healthcare, municipal systems, and the community to facilitate access to services, address barriers to care, increase the delivery of culturally appropriate services and actively build individual and community capacity. This position will be assigned to work in the communities primarily within CHA’s catchment area and work to improve community resilience and address identified health priorities and Social Determinants of Health challenges of patients and community members. The CHW will be embedded in CHA clinics, community agencies, and in the local Boards of Health within CHA’s catchment area. The CHW helps strengthen CHAs response efforts to health concerns and addresses existing health disparities. The CHW executes programs that support and empower communities and health systems in equitable access to healthcare services, and equip partners to handle future emergencies, such as pandemics, in ways that mitigate inequities. Responsibilities of the CHW will be consistent with the scope of practice acts governing community health workers in Massachusetts.

    Responsibilities

    + Conducts SDOH screening and, when appropriate, provides and documents referrals along with additional support to meet the identified gaps

    + Receives referrals for patient follow-up and services

    + Provides follow-up with patients, as needed, and documents the outcome of referral(s)

    + Conducts basic social needs assessments for patients referred by primary care teams. Identifies patient strengths and abilities. Adapts style to engage patients' in appropriate levels of problem-solving and self-advocacy

    + Provide resources and support to assist patients in completing paperwork and applications, including applications for: transportation, SNAP benefits, housing subsidies, other public benefits, etc. to build community and patient resilience

    + Collaborates with a multi-disciplinary team to develop a comprehensive list of initiatives, contacts, and partners, of health and human services providers and maintains working relationships with these service providers that may include advocacy organizations, municipalities, non-profits and faith organizations, and other groups who address social determinates of health

    + Facilitates or conducts periodic in-services for staff, patients, community partners, and community members on a range of issues related to, but not limited to, health access, nutrition, well-being, healthy living, active living, health prevention, and chronic diseases.

    + Facilitate workshops on a broad range of program topics, including but not limited to health access, nutrition, mental and physical well-being, healthy living, active lifestyles, disease prevention, and the management of chronic conditions.

    + Conducts outreach at internal and external events, health fairs, and other public/selective gatherings

    + Engages interpreters when necessary to provide information in the patient's language of care.

    + Completes all necessary patient-related documentation in a timely manner in EPIC, when appropriate and according to CHA policies and procedures.

    + Completes all necessary client-related documentation in the appropriate format (RedCap, Excel, etc) when providing services to non-CHA clients.

    + Tracks open, inactive, and closed cases and maintains complete records of education and referrals.

    + Completes all administrative paperwork (e.g. monthly and quarterly reports, timesheets) in a timely manner.

    + Works collaboratively with other providers, staff, community partners, patients and the community

    + Works in various locations based on programmatic needs

    + Represents CHA at community health fairs, workshops, and other public forums.

    + Conducts outreach and engagement in the community at events, health fairs, workshops, conferences, and other community/public forums and may have alternative hours.

    + Ability to multitask and effectively interact with all members of the health care team, including patients.

    + Attends all required training sessions and monthly meetings as required by funding and as determined by the supervisor.

    + Develops appropriate education and outreach materials on various topics

    + Assigned specific tasks, based on the population being served

    + Other duties as assigned.

    Requirements

    + College degree in health science, social science or health preferred or 3 yrs of relevant community work experience in the program topic area

    + Licensure: Community Health Worker Certification or Certification eligible

    + Requires ability to travel between multiple sites such as primary care sites, hospitals/ specialty care, or community locations.

    + Work Experience: Minimum of two years experience in community health worker role required.

    + Experience with multi-cultural and multi-lingual populations is required.

    + Has a clear understanding of the focus population being served while in the position

    + Shared lived experience similar to population/program topic served preferred

    + Bilingual (English and Portuguese, Haitian Creole or Spanish) is strongly preferred.
