**Pay Range:** $40.75 - $72.94 per hour

Summary:
The Dialysis Access Coordinator RN works closely with transplant surgeons, interventional radiologists and nephrologists, nephrology providers and dialysis unit staff, and other care team members, as well as patients and families to educate and facilitate the placement and use of the optimal patient access option for dialysis treatments. This may include either vascular access or peritoneal dialysis (PD) catheter access.
The Dialysis Access Coordinator RN serves as a role model for other clinical nursing staff and collaborates with the dialysis access team. The Dialysis Access Coordinator RN exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures. The Dialysis Access Coordinator RN is accountable to demonstrate and continue to develop proficiency (both knowledge and skill) specific to all types of support for patients in need of vascular access procedures.

Required Job Qualifications:
• Current State of Illinois Registered Nurse licensure
• Baccalaureate degree in nursing.
• Minimum 6 months of outpatient healthcare nursing experience.
• Knowledge of current access devices.
• Basic vascular ultrasound knowledge/experience desirable.

• Proficient in optimizing workflows through technology in Epic and MS Outlook.

• Main contact for all scheduling and coordination of vascular access patients under care at the University Transplant Program (UTP).
• Communicates and collaborates with multidisciplinary dialysis access team to determine and maintain the safest dialysis access for patients.
• Remains knowledgeable of all UTP dialysis access patients’ condition and needs.
• Develops close relationship with dialysis clinic managers, access coordinators, and staff nurses.
• Coordinates calls from dialysis clinics for procedure scheduling and any patient care concerns.
• Able to triage contacts from nephrologists, dialysis units, and patients/families regarding concerns about existing dialysis access, including ability to identify potentially emergent situations and mitigate and direct patients/families or other care providers as indicated.
• Tracks UTP access patients to ensure follow-up appointments are made and rescheduled if missed.
• Facilitates communication and care coordination with hospital dialysis access stakeholders, including nephrology team, interventional radiology, and interventional radiology and others as indicated.
• Communicates with inpatient RN coordinators to track UTP dialysis access patients admitted to the hospital and facilitates follow-up and additional testing and procedures as indicated.
• Documents individualized plans of care related to dialysis access developed by the multidisciplinary dialysis access care team. Facilitates multidisciplinary dialysis access care rounds.
• Provides education to patients and families about dialysis access options, procedures, and care, taking into account specific, physical, psychosocial, and learning needs of each patient/family. Advocates for dialysis catheter avoidance/reduction.
• Facilitates clinic evaluation of dialysis access patients, including collecting care data, collating prior medical records including operative reports and imaging studies, and obtaining reports from dialysis units regarding patients’ status on dialysis, clinical concerns, and dialysis adequacy.
• Able to communicate with surgical providers in a clear and succinct fashion.
• Routing cleaning of ultrasound machine and escalating supply ordering needs related to the ultrasound machine effectively to leadership.
• Assist with office procedures in dialysis access patients, including knowledge of sterile technique and common office procedures. Inventory and re-order supplies as required.
• Excellent documentation skills, including ability to upload images, faxes, and other documents sent from referring providers into the media tab of the electronic medical record and communicates receipt of materials with dialysis providers.
• Facilitates clinic flow, including orderly progression of schedule and continuity of individual patient care.
• Provides education to dialysis units regarding patient-specific dialysis access concerns.
• Facilitates pre-operative clearance (as indicated) and surgical scheduling of UTP access patients, including having sufficient knowledge of dialysis access procedures to assist in consenting patients and families for dialysis access procedures.
• Administrative coordination, including faxing operative notes to appropriate clinics and providers, contacting patients/clinics regarding test results, or advising patient of surgery times/location.
• Provides education and supervision of other providers within UTP who are part of the UTP dialysis access care team.
• Guides others through the use of formal/informal communication systems, including collaborations across colleagues in highly multi-disciplinary departments.
• Takes appropriate nursing action in emergencies.
