Care Transition Specialist (CTS)

• **Work location:** Texas Health Ft. Worth, 1301 Pennsylvania Avenue, Ft. Worth, TX 76104

• **Work hours:** Full-time, 40 hours/week; Monday to Friday Between 7:00AM-4:00PM, 8-hour days, start time will vary - either at 7:00AM or 7:30AM

Here’s What You Need

• High School Diploma or Equivalent Required

• **3 Years** relevant experience **Required** , ( **preferably** in a healthcare or service environment)

• Knowledge of medical terminology preferred

+ Problem solving skills

+ Latitude for decision making and setting of priorities

• Interpersonal skills and Communication skills

• Ability to work with minimal supervision or without supervision when necessary

• Proficient computer skills with experience in: MS Outlook, MS Office (Word, Excel), internet navigation, clinically based software, and financially based software

What You Will Do

* Responsible for coordinating the clerical and administrative processes for entity care transitions management

* Responsible for managing departmental mailbox, phone lines and/or fax

* Supports transition planning activities as needed under the direction of care transition managers: Calls vendors, physician offices, clinics, etc.; Faxes (electronic and traditional) documents as needed

* Schedule follow-up appointments, call Insurance companies for referral approvals for patients

* Assist/support the Case Managers – will be assigned to 3 to 4 floors of patients – will go into the patient’s rooms for signatures on documents, as well as deliver the Durable Medical Equipment/DME (ie. Wheelchairs, walkers, wound vacs) to the patients prior to discharge

* Makes copies or gathers other data as needed

* Collects and enters data as needed

* Submits for prior authorizations for transportation and post-acute disposition via insurance portal.

* Completes other tasks and projects as assigned
