Job Overview
The **Nurse Manager** for Meadows on Fairview TCU located in Wyoming, MN is a RN who is responsible for continuity of nursing care by delegating, directing, and monitoring nursing care delivered. Demonstrates management skills through effective problem solving and decision making, assuring complaiance with State and Federal regulations as well as Ebenezer Quality standards.

**Meadows on Fairview TCU** is located in the city of Wyoming, MN, steps away from the Fairview Lakes Hospital and a short distance from Forest Lake and North Branch. We offer a home like atmosphere where you feel like you belong the minute you arrive! Our 14-bed Transitional Care Unit is small enough to get to know your co-workers and residents yet large enough to provide you with opportunities and room to grow.
Nurse schedule includes:
+ 64 hours every two weeks
+ Day shift
+ Weekends as needed

Job Responsibilities:
+ Leads, supervises, directs and/or delegates all nursing functions on assigned unit or floor to assure that care is provided per individualized plan of care. Assures staff compliance with State and Federal regulations, Nursing Standards of Practice and Ebenezer Quality Assurance Standards.
+ Responsible for designed unit
+ Assures that reporting process is accurate, up to date, and provides continuity of care from shift to shift
+ Observes and evaluates cares given by nursing staff through daily rounds, random auits
+ Assures necessary equipment is present
+ Assure room placement is appropriate
+ Assures that medications and treatments are administered per physician orders and facility policies
+ Reviews preadmission assesments as needed
+ Delegate tasks/duties to nursing staff within their scope of practice
+ Evaluates staff performance and address concern promptly and appropriately
+ Conduts staff performance reivews & other surveys
+ Coordinates team meetings
+ Assures that plan of care, medical records and other documentation are up to date
+ Coordinates the Admission and Discharge process
+ Develops and/or assures accuracy of care plan process
+ Actively participates in IDT meetings and care conferences
+ Audits patient records to ensure Code Status and POLST match
+ Assures that changes in condition or acure episodes are reflected in the medical record/plan of care
+ Assures that resident family and physician are notified of any changes in plan of care
+ Investigates and follows-up on all invident reports, medications errors, and Vulnerable Adult reports
+ Actively participates in audits to assure quality of care and practices
+ Reports problematic areas to management and provides suuggestions and solutions. Makes decisions regarding situations, implements actions and evaluates outcomes
+ Offers feedback to Administrator and DON regarding area of concern
+ Serves as positive role model to staff
+ Educates nursing staff as needed
+ Participates in QAPI and quality improvements projects
+ Serves as facility supervisor and assumes administrative responsibilities as assigned
+ Serves as emerency contact/lead for the facility for all departments
+ Monitors delivery of care throughout the facility
+ Conducts tours for families and potential residents as requested
+ Displays ingenuity in anticipating and meeting the challenges of unexpected situations
+ Participates in weekend rotation as designated by Administrator of facility
+ Participates in discipline/corrective action process. Disciplines nursing staff with input from Director of Nursing and HR
+ Able to send staff home regarding VA and performance issue investigations
+ Assume administrative responsibilities in the absence of the Administrator and the Director of Nursing when assigned
+ Serves as lead in all emergency situations as situation warrants
Required:
+ Vocational/Technical Training or Associate Degree - RN
+ RN Licensurewith the MN Board of Nursing
+ Current Basic Life Support (BLS) for Health Care Provider from American Hearth Association (AHA) or American Red Cross
+ 1-3 years of experince - LTC
Preferred:
+ Bachelor's Degree
+ 3-5 years of experience - LTC

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
