RN Care Manager - Full Time - Eddy SeniorCare (Latham)
Eddy SeniorCare has a rewarding opportunity for an RN Case Manager to work out of our 385 Watervliet Shaker Road, Latham location!
PACE is a unique program on the forefront of trends in healthcare, combining an insurance role (we are the Medicare/Medicaid insurer for our participants) with a comprehensive healthcare provider role (primary care, home health, adult daycare, transportation, all coordinated by an interdisciplinary team).
The primary purpose of your job position is to promote and maintain the health of participants enrolled in Eddy Senior Care through teaching, counseling, and appropriate preventive services. The Registered nurse (RN) is responsible for assessment of health needs and implementation of nursing care plans for these participants. The RN provides direct nursing care to the participants, supervises the day-to-day nursing activities including care provided by home health aides. Such supervision must be in accordance with current state, federal and local standards, guidelines, and regulations that govern our facility, and as may be required by the Nurse Manager of Clinical Operations, to ensure that the highest degree of quality care is maintained at all times.
This position is a community based HomeCare RN role, providing hands-on nursing care to our participants in their homes
Work Schedule:
+ Monday to Friday 8am to 4:30pm
Requirements:
+ NYS RN licensure.
+ Two or more years experience.
+ Experience and desire to care for seniors.

Pay Range: $36.50 - $49.95
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
