    Job Opening: LPN – Home and Community (Hospice) | Per Diem | Days

     

    **Pay Range: $28.02 – $42.04 per hour** _(Hiring rate based on experience, education, and other factors)_

     

    PeaceHealth is seeking a compassionate and skilled **Licensed Practical Nurse (LPN)** to join our Home and Community Hospice team in a **Per Diem/Relief, Day shift** role.

    About the Role:

    In this position, you’ll work under RN supervision to provide direct nursing care to hospice patients in the comfort of their homes. You’ll support care plan implementation, administer medications, monitor conditions, and educate patients and families—making a direct impact during sensitive, critical times.

    Details of the position:

    + Deliver personalized care to patients in home settings

    + Administer medications and manage wound care, drains, ports, and PICC lines

    + Report changes in condition and collaborate with healthcare teams

    + Educate patients and families on disease management

    + Maintain accurate and timely documentation

    What you bring:

    + Graduation from an accredited LPN program

    + Active LPN license in Oregon

    + Current BLS certification

    + Valid driver’s license and proof of insurance

    + Minimum 1 year of LPN experience preferred

    + Ability to work independently and communicate effectively with patients experiencing complex health and emotional challenges
