Located on the campus of Scripps Mercy Hospital San Diego, Scripps Ambulatory Clinical Pharmacy Services is a system-wide team of Advanced Practice Pharmacists. This position would provide top-of-license clinical management to our Neurology patient populations, but may also support Osteoporosis, Primary Care or HIV. Understanding that the treatment of diseases extends beyond just medications, we focus on lifestyle and prevention counseling, in addition to the pharmacologic aspects of care. The aim is to get to know what is important to our patients, define a set of goals, and work together to achieve those goals and enhance a person’s overall quality of life. Scripps Ambulatory Clinical Pharmacy Services collaborate with physician specialists to assist with improving the long-term health and quality of life of our patients. The Advance Practice Pharmacist is key to providing care as it relates to patient medication needs.

In order to remain eligible for your retention incentive the following criteria must be met :

* Must remain in original hired FTE Status and Shift (if specified in offer letter)

* Transfers to other locations will be reviewed on a case-by-case basis and may result in forfeiting remaining incentive bonus unless specifically noted in your offer letter.

*Required Education/Experience/Specialized Skills:*

* BS Pharmacy or Pharm.D. Degree

* PGY1 and PGY2 residency in area of specialty or related specialty and 1 years' experience OR PGY1 residency and 3 years' experience in Clinical pharmacy practice (inpatient or ambulatory) in area of specialization or related specialty OR 5 years' experience in area of specialization.

* Practice experience must include at least 1 year working under collaborative practice agreements or health system protocols.

**

*Required Certification/Registration:*

* California Pharmacist License

* AHA BLS for the health care provider and ACLS certifications required prior to being trained to respond to codes.

* Board of Pharmacy Specialties (BPS) or other nationally accredited certification required within 2 years' of hire.

* Must be eligible to apply for the Advance Practice Pharmacist license within 1 year of hire and obtain license within 2 years' of hire.

*Preferred Education/Experience/Specialized Skills/Certification:*

* Experience in a multi-site health system pharmacy enterprise

* Teaching certificate and/or experience precepting students and/or residents
