    BayCare HomeCare is currently in search of our newest **Home Health LPN Per Visit** who is passionate about providing outstanding customer service to our community. We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area.

     

    + **Facility:** BayCare HomeCare

    + **Location:** Bradenton, FL

    + **Status:** **Per Visit**

    + **Shift Hours:** 8:00 AM - 5:00 PM

    + **Weekend Work:** 1 Weekend Rotation a month

    + **On Call:** Yes

    + **Home visits in assigned service area:** **Manatee County, FL**

     

    The **Per Visit Home Health LPN** provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. **Responsibilities include:**

     

    + Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse

    + Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse

    + Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status

    + Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

    Required Certifications and Licensures:

    + BLS (Basic Life Support)

    + LPN (Registered Nurse) License

    + Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

    Education:

    + Required Associate's or Diploma Nursing

    Experience:

    + Required 1 year Licensed Practical Nurse

    + **Preferred 1 year Home Care**
