Description:
The Women’s Health Navigator provides patient care coordination by serving as a liaison between the patient and the practitioners and services she may require. The Women’s Health Navigator acts as a patient advocate, provides patient and family education, and coordinates all aspects of care to assure the provision of seamless comprehensive services. The Women’s Health Navigator plays a critical role in the patient experience including community outreach, optimizing patient services and working collaboratively with the team to create a patient centered environment. This involves developing expertise in the operations and systems of the enterprise related to promoting and maintaining health and wellbeing in women. The scope of navigation extends beyond the services provided within the CWHC and is intended to coordinate care across the continuum in a comprehensive way. The role of the Women’s Health Navigator is an integral part of the organizations commitment to provide holistic patient centered care for women.
**You Will Be Responsible For** :
+ Clinical Practice: Assesses physical, psychological and educational needs of patients according to guidelines, standards of care and individual needs.
+ Assesses patient care needs and/or changes in condition throughout treatments and procedures and communicates findings to the provider.
+ Planning: Coordinates plan of care for patients, including, but not limited to, appointments, transportation, education, and follow up treatments to ensure timely delivery of diagnostic and treatment services.
+ Coordinates referrals to medical and radiation oncology, physical therapy, and other services as determined by providers.
+ Provide and coordinate education and information to patient and family to ensure a seamless, continuous, and comprehensive continuum of care.
Work Hours/Shift:
+ 40 hours, days
**Requirements** :
+ Current Connecticut RN Licensure.
+ Bachelor of Science in Nursing (BSN).
+ Minimum 2-4 years’ previous experience in a healthcare related field
+ Must have certification for CN-BN within 12 months of accepting this position, as required by NAPBC
+ This position requires a person with a positive attitude who is self-directed, pleasant, cooperative, assertive yet compassionate displaying a professional demeanor with patients, families, physicians and fellow workers.
