    Position OverviewUnited Health Services (UHS) is seeking a Licensed Practical Nurse (LPN) to join our dedicated care team in Candor, NY. In this role, you’ll deliver direct patient care, support daily clinical operations, and help ensure a safe and compassionate environment for those we serve. Your clinical skills and attention to detail will be essential in providing high-quality care in a close-knit, community-focused setting.

    Primary Department, Division, or Unit:

    UHS Primary Care Candor

    Work Shift and Schedule:

    This is a per diem position, which means you will work on an as needed, agreed upon basis. Working hours will be assigned by your manager.

    Compensation Range:

    $22.97 - $34.46 per hour, depending on experience

     

    This position is not eligible for benefits.

    Job Responsibilities:

    + Perform clinical tasks such as taking vital signs, collecting specimens, and assisting with procedures, always within the LPN scope of practice and under supervision when required

    + Administer medications safely and accurately, utilizing knowledge of pharmacology and following UHS policies and current clinical standards

    + Educate patients and their families on health promotion, disease prevention, and management strategies, as directed by a provider or registered nurse

    + Support and ensure compliance with all regulatory standards including Joint Commission, NYS Department of Health, and infection control protocols

    + Perform CLIA-waived laboratory testing, including quality control procedures, documentation, and participation in tri-annual AAFP proficiency testing

    + Maintain accurate and timely documentation in the electronic medical record system to support patient safety and care coordination

    + Participate in department-specific credentialing and lab validations as required by practice standards and regulations

    + Collaborate with providers, RNs, and the broader care team to coordinate patient care and promote positive health outcomes

    + Assist with front office functions such as scheduling, patient intake, and communication of clinical instructions when appropriate

    + Actively engage in Performance Improvement (PI) initiatives by reviewing data, offering feedback, and implementing changes to enhance care quality

    + Maintain clinical competency by participating in ongoing training, in-services, and educational opportunities

    + Uphold a culture of patient-centered care by contributing to a safe, respectful, and welcoming environment for all individuals

    Position Qualifications:

    + Minimum Required:

    + Graduated an accredited school of Practical Nursing

    + Current New York State Practical Nurse License, a Limited Permit, or ability to practice within the 90 day grace period allowed by New York State guidelines

    + Preferred:

    + Basic Skills Certifications (CPR)
