Description
Aftercare and permanency planning begins within 72 hours of a youth’s placement in a Qualified Residential Treatment Program (QRTP). The Aftercare DSP will aid youth and their discharge resource with permanency and discharge planning while in the ECC’s QRTP setting. In addition, the Aftercare DSP will assist youth and their discharge resource by helping them become integrated and engaged in their community as they are being discharged from ECC’s QRTP.
CFTSS (Children and Family Treatment and Support Services) offers opportunities to better meet the behavioral health needs at earlier junctures in a child/youth’s life to prevent the onset or progression of behavioral health condition. CFTSS is a set of services designed to help youth and their families gain the necessary tools to be successful in their homes and communities. This is done through goal driven supports and solution based interventions.
The Aftercare DSP will meet the Family First Prevention Services Act (FFPSA) Aftercare Services requirements and meet with youth both during placement and post discharge. Once discharged the Aftercare DSP will continue to meet FFPSA requirements while also providing Community Psychiatric Support and Treatment Services (CPST) a service under the CFTSS umbrella post- discharge from the QRTP setting. Generally, CPST activities aim to strengthen the coping capacity of a youth and their family; and to regain stability in their everyday functionality after a crisis. Specific activities include developing interventions aimed at building resilience, strengthening family bonds, and relationship building in the academic, home, workplace, community and other settings where the youth spends their day.
Responsibilities:
+ The Aftercare DSP will provide provision of aftercare services and ensure the youth and family/discharge resource’s needs are being met.
+ Monthly Face-to-face Requirements: face-to-face mandates are as followed- at minimum there are to be 2 substantive face-to-face contacts with youth per month (at least one of the two contacts must be held at the youth's place of residence the other can be in the community). 1 substantive face-to-face contact with the family/discharge resource per month (this contact can include the youth but must be held at the youth's place of residence).
+ Permanency Planning Team Meetings: Responsible for holding monthly meetings regarding youth’s success/barriers related to discharge planning in the residential setting and 2 times during the 6-month aftercare duration post-discharge. Aftercare DSP will collaborate with other members of the Permanency Planning Team to promote success.
+ During the 6-month aftercare post-discharge timeframe, the Aftercare DSP is responsible for providing the following array of CPST services to the youth and family: Intensive Interventions (Counseling), Crisis Avoidance (Counseling), Intermediate Term Crisis Management (Counseling), Rehabilitative Psychoeducation, Strengths Based Service Planning, and Rehabilitative Supports.
+ Availability: Flexibility to create your own schedule depending on client availability but must have availability to provide services during after-school/evenings if needed.
+ Documentation: Complete required documentation for Medicaid billing within prescribed timeframes. All case notes and casework contacts must documented in OCFS CONNECTIONS database and EMR Netsmart.
+ Report to Aftercare Coordinator/Clinical Supervisor for regularly scheduled supervision to review cases, youth and family needs and service progress and/or barriers.
+ Collaborate with various service providers which will included but not limited to; LDSS County Case Workers, other QRTP Agencies, Schools, Community Based Providers, etc. The Aftercare DSP will be responsible for making additional referrals in the community based on the youth and family/discharge resources needs.
Qualifications
All Applicants much haveoneof the following:
+ Must have a Bachelor Degreeandat least 3 years’ experience in mental health/human services field or working with youth and families
OR
+ Master’s degree (Social Work, Marriage and Family Therapy, Psychology, Mental Health Counseling, etc.)

+ Computer skills, including Microsoft Word and Outlook.
+ Self-confidence and an ability to work with children and young adults with a variety of needs.
+ Effective problem-solving skills.
+ Must be reliable and able to work independently and understand the importance of maintaining confidentiality/communication with the multidisciplinary team.
+ Must be cleared by the NYS Justice Center through fingerprinting for a Criminal History Records search.
+ Must be cleared for work through Drug Testing.
+ Must be cleared by the NYS Office of Children and Family Services for instances of child abuse and/or neglect.
+ Valid, unrestricted Driver’s License, with reliable/safe transportation.
+ Acceptable clearance checks for State Central Register and Driver license as required by the program.
+ Successful completion of mandatory training.
