SUMMARY The MDS Coordinator Licensed Vocational Nurse (LVN) is responsible for coordinating the Minimum Data Set (MDS) process, which includes assessing, documenting, and ensuring the accuracy of residents' care needs. This information is used to develop individualized care plans, support quality outcomes, and ensure compliance with regulatory and reimbursement standards. Key responsibilities include organizing and coordinating Interdisciplinary Team (IDT) meetings, interviewing residents, and completing quarterly, annual, and significant change assessments. The MDS Coordinator LVN is also responsible for carrying out all medical doctor (MD) orders as needed to support resident care and treatment. In addition to MDS-related duties, this position may be asked to assist with medication administration and direct resident care as needed, to ensure continuity of high-quality care A minimum 60-day notice is required upon termination of employment from this position to allow for adequate training and a smooth transition of responsibilities.

EDUCATION/EXPERIENCE
Graduate of an accredited school of nursing.
Preferred 5 years of clinical experience as an LVN in an acute care or skilled nursing facility.
2+ years MDS experience (preferred)
CERTIFICATES, LICENSES, REGISTRATIONS
Current license to practice Vocational Nursing in the State of California.
MDS Certificate or Training Certificate (Required)
Current CPR certificate.
ACLS desired.
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure
manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees or organization.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rate, ratio and percent to draw and interpret bar graphs.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
OTHER SKILLS AND ABILITIES
Ability to adapt to a varied work schedule and high patient load.
Ability to provide care to all age ranges from pediatrics to geriatrics.
Good communication and customer service skills.
