Summary of Primary Function/General Purpose of Position (2-3 sentences)
The Nurse Triage Specialist answers incoming calls to triage the severity of patient symptoms as presented by the caller and directing the patient to the appropriate level of care by connecting them to the appropriate services, including sending messages to the clinical team, transferring patients to appropriate Bon Secours Mercy Health and community resources, pending prescription refills, and coordinating with hospital ancillary scheduling. The Specialist is responsible for accurately scheduling and registering physician appointments when clinically appropriate. The Specialist is responsible for completing an accurate clinical assessment, based on information provided by the caller, and documenting the assessment, utilizing the nurse triage algorithms. Each team member is encouraged to utilize their critical thinking skills when performing the triage function as appropriate for their level of training and presentation of symptoms / complaints as presented by the caller. Triage services will be made available through the call center switchboard for Bon Secours Mercy Health for patients who are established with our medical oncology program. All encounters will be documented in the EMR as well as other tools as identified. Specialist will assist with the evaluation and review of all incoming ambulatory oncology referrals. Specialists may be asked to assist with additional functions within the Bon Secours Mercy Health Oncology as deemed necessary and appropriate.

10-20% of Time - Triages incoming patient calls via the Nurse Triage phone lines and directs patients to the appropriate resources of care as needed and documents all interactions in EPIC. Utilizes appropriate Bon Secours Mercy Heath resources necessary to service the patient per protocol and policy to ensure patients have access to the resources needed for safe continuity of care.
10% of Time - Schedules patient for physician practice appointments, per protocol and policy.
20% of Time - Meets the needs of the patient, including pending prescriptions refills, sending messages to the clinical staff, transferring to Bon Secours Mercy Health and community resources, and coordinating with hospital ancillary scheduling.
50-60% of Time - Triages incoming hematology and oncology referrals. Determines appropriateness referral, references new patient referral guidelines, completes chart abstraction, evaluates need for additional records, assists with scheduling, and communicates with oncology support staff as needed.

Minimum Qualifications
Minimum Years and Type of Experience:
3 years of experience in clinical nursing in the hospital or physician practice setting.
Other Knowledge, Skills and Abilities Required:
Experience in oncology and patient triage preferred.

Comfortable with phone and telehealth communications, EMR utilization.
