Job Summary: The Medical Director, Population Health (H) serves as the physician leader for Saint Francis Health System’s (SFHS) Population Health initiatives. This role provides clinical oversight and guidance to Population Health’s quality, care management, and clinical redesign efforts while serving as a key member of the Population Health management team.
Minimum Education: Commensurate with obtaining a Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
Licensure, Registration and/or Certification: Current physician licensure in State of Oklahoma.
Work Experience: Minimum 5 years of experience in patient care within a medical practice. Previous medical leadership, population health, or value-based payment programs experience, preferred.
Knowledge, Skills and Abilities: Demonstrated financial acumen and experience with quantitative financial and clinical results, strategic review, critical thinking and decision-making skills. Ability to build effective partnerships, influence other leaders, and interface with functions at all levels within the organization. Working knowledge in principals of population health improvement, quality improvement, and public health issues. Ability to work independently and collaboratively in a fast-paced environment, managing multiple priorities with competing deadlines. Ability to navigate and drive transformation across diverse clinical operational teams. Skilled at identifying complex care challenges and implementing innovative, evidence-based solutions.
Essential Functions and Responsibilities: Contributes as part of the Population Health management team in strategic planning to achieve goals and objectives. Actively engages in understanding industry trends, marketplace dynamics, and government relations and promotes practice of evidenced-based medicine and population health. Serves as the physician and clinical executive contact at Saint Francis Population Health, and works collaboratively with health plans, participating community providers, various internal stakeholders (e.g. Saint Francis affiliate hospitals, Warren Clinic providers), and business partners. Assumes a leadership role to influence management of quality and clinical outcomes. Collaborates with various internal and external stakeholders to support the development and management of provider networks supporting Population Health initiatives. Provides clinical direction and oversight to Population Health's clinical teams including quality, care management, pharmacy, risk adjustment program, and post-acute care initiatives. Reviews quality and utilization reporting to identify areas for performance improvement. Develops and implements strategies to improve health outcomes and reduce costs across patient populations. Promotes evidence-based practices and clinical protocols to enhance patient outcomes. Drives the performance of quality outcomes and utilization/cost performance of populations covered under Population Health programs and value-based contracts, to include but not limited to, Community Care of Oklahoma, Medicare ACO, Saint Francis’s employer-sponsored health plan.
Decision Making: Independent judgment in making decisions from many diversified alternatives that are subject to general review in final stages only.
Working Relationships: Coordinates activities of others (does not supervise). Leads others in same work performed (does not supervise). Has input on performance evaluations but does not prepare or give. Works directly with patients and/or customers. Works with internal and/or external customers via telephone or face to face interaction. Works with other healthcare professionals and staff. Works frequently with individuals at Director level or above.
