• Case Manager 12th Street evenings

    Are you a caring human services professional looking for part-time hours? Unity House of Troy’s Transitional Apartment Services Program (TAS) 12th Street has openings for a part time evening case manager, Thursday - Saturday 5:00PM - 10:00PM

     

    Position is eligible for a pro-rated sign on bonus

    Responsibilities:

    Manage day-to-day routines and daily resident care coordination.

     

    Oversight of medication supervision room and medication inventory.

     

    Engaging in ongoing, regular interactions and providing supportive services to program consumers.

     

    Assist consumers with severe and persistent mental illness.

     

    Maintaining charts and documents in keeping with Office of Mental Health guidelines.

     

    Assisting with daily living skills including light to moderate cleaning as needed.

     

    Communicating with providers on a regular basis.

     

    Promote a culture of belonging and inclusion.

    Requirements:

    High School Diploma and at least two or more years working with people with mental health issues. Experience working with multiple populations preferred. Valid and clean NYS driver's license. Reliable transportation.

     

    Unity House is a Rensselaer County-based human service agency that provides a wide range of services to meet the otherwise unmet needs of people in our community who are hurting and struggling.
