    Your Specific Responsibilities:

    The HIV Community Liaison is a highly professional position with lead responsibilities for community focused HIV/AIDS prevention, treatment care and early intervention, community planning, testing, risk assessment, education, and training (HIV 500/501 and others as needed) activities. Provides and coordinates technical assistance, guidance, and consultation for the same subjects to internal and external partners. It is responsible for serving Area 3 and 13 (Alachua, Bradford, Columbia, Citrus, Dixie, Gilchrist, Hamilton, Lafyette, Lake, Levy, Marion, Sumter, Suwannee, Putnam and Union counties). This position is responsible for working with community partners and communities with the highest prevalence of HIV/AIDS to initiate community mobilization/engagement initiatives and community planning. The position will work under the direct supervision of the Area 3/13 HIV/AIDS Health Services Manager A.

     

    Incumbent performs regular site visits to each Area 3 and 13 county health department (CHD) to coordinate community outreach initiatives. Work with community partners and effected communities to initiate community mobilization/engagement initiatives. Formulates linkages between the local CHD, local colleges/universities, local providers, and local businesses to provide technical assistance and capacity building to community-based organizations (CBO).

     

    Promote strategies to reduce HIV/AIDS stigma and discrimination. Provide technical assistance to viable community agencies. Work with faith-based organizations to strengthen their capacity to meet the HIV/AIDS needs of communities statistically most impacted. Assist in mobilizing congregations and communities to respond to the HIV/AIDS crisis among communities with the highest prevalence of HIV/AIDS. Increase HIV/AIDS efforts in high and moderate HIV/AIDS effected communities and high-risk areas of Area 3 and 13.

     

    Increase understanding of communities’ social, economic, and political landscape to assist in addressing HIV/AIDS. Assist the community with knowledge of biomedical tools to prevent HIV transmission. Provide technical assistance to providers to increase post-test counseling for community agencies and ensure providers repeat testing for high-risk populations. Provide technical assistance to community CHDs, CBOs, and providers regarding PrEP treatment as prevention. Collaborates with the HIV testing providers in identifying gatekeepers in their priority demographic. Identifies where providers, community organizations, and outreach events need to be to reach their key demographic (sites frequented by an identified high risk-populations). Assist with creating a resource list of agencies and community partners that have the capacity and expertise to provide an array of HIV/STD prevention services. Assist in establishing public/private partnerships within disproportionately impacted communities. Participate regularly in health fairs, community screenings/meetings/events, and HIV/AIDS educational presentations as requested by community partners. Conducts awareness day events. Collaborate with CBOs and community leaders to assist in identifying locations, times, and events to create a successful awareness event. Identify racial/ethnic minorities for priority population testing. Responsible for initiatives, including social marketing, directed to the faith, and disproportionately identified high-risk communities, to include technical assistance to and providing support Participates directly in community planning of health fairs, community outreaches and HIV/AIDS educational presentations as requested by community partners. Develops and distributes approved and current HIV/AIDS information, educational materials, and statistics in the community. Creates and implements counseling & testing, surveys, focus groups and/or community assessments for identified high-risk populations.

     

    In the absence of the EIC, acts as back-up for the Early Intervention Coordinator (EIC) for the following duties: maintain adequate quality assurance documentation related to registration of new HIV testing sites, annual re-registration compliance of existing HIV testing sites, annual Memorandums of Agreement (MOAs), and counseling and other reporting data as appropriate. Conducts 500/501 trainings and updates for CHD and non-CHD sites, in-services, and trainings for CBOs, rapid testing trainings and other applicable trainings for HIV prevention and education. Ensures that those who completes 500/501 and 501 update courses are following policies and procedures and are meeting post-test counseling goals and certification. Maintains documentation of training evaluations and logs for trainings.

     

    Records, certifies, and submits EARs, CSRs, and timesheets per DOH policy within specified policy time frames. Participates in department, local/statewide, training, staff, and unit meetings. Performs other related duties as assigned or when required. Administrative and lead duties including but not limited to prepares and maintains timely and accurate records including work plans and submit all necessary reports on time according to program guidelines and requirements. Maintain professional working relationships with other departments, agencies, and community members. Maintains client confidentiality and security in accordance DOHP50-10-10 Information Security & Privacy Policy Confidential Information.

    Required Knowledge, Skills, and Abilities:

    Knowledge and experience working with HIV/STD Programs.

     

    Knowledge of HIV disease process, treatment, prevention measures

     

    Knowledge of Ryan White Programs and/or other HIV/AIDS Patient Care Programs

     

    Knowledge of minority communities and experience working with diverse populations.

     

    Knowledge and skills conducting community mobilization.

     

    Skills as a team builder and leader

     

    Ability to communicate with variety of audiences in verbal and written forms.

     

    Ability to work independently.

     

    Ability to create presentations and give health education presentations to diverse audiences (minimum one year experience)

     

    Ability to educate stakeholders in HIV/AIDS topics.

     

    Ability to use MS Office products.

     

    Ability to travel

    Qualifications:

    Minimum -

    • Work experience in a healthcare or human service field or an associate degree in a healthcare or human service field

    • Driver’s License , if out of state a Florida Driver’s License is required within 30 days of hire

    • Required to travel out of county, into varied socioeconomic neighborhoods, out of county travel, 25-50% of time per week.

    • Minimum of one year experience working in an HIV/STD testing and/or counseling position, working at disease prevention related outreaches, and/or patient coordination/navigation.

    • Minimum of one year experience in community engagement, public speaking , or giving community presentations

    • Willing to periodically work before and/or after work hours, including weekends to organize and participate in outreach events.

    • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

    Preferred -

    • Bachelor’s Degree or higher in a healthcare or human service field.

    • One or more years in a lead position

    • Bilingual or multilingual -English/Spanish and/ or Haitian Creole
