    Description

    The Preventive Services Primary FTC Facilitator is responsible for scheduling and facilitating conferences for families participating in the Prevention Program at Astor within the Family Connections Program. All conferences will be conducted using a strength based perspective with a lens of safety and well-being for the children and family members. The FTC Facilitator must be able to distinguish safety from risk, and support the conference participants in reaching a consensus that meets the presenting needs/concerns and programmatic goals of the family.

    Responsibilities:

    + The FTC Facilitator will facilitate all Family Team Conferences (FTC) for cases carried by all Case Planners in the Preventive Program.

    + Attend monthly ACS FTC Facilitator meetings at ACS. Report on any new information and advocate appropriately for the Astor Prevention program.

    + The FTC Facilitator will utilize a strength based, family-centered approach to conducting conferences with families and other stake holders in order to ensure families are planning towards making positive change, and that cases move timely and appropriately towards closure.

    + Ensure that solutions and/or next steps are measurable, have time limits and are matched to needs of family and service providers to accomplish next steps.

    + Facilitator is responsible for confirming appointments with families, 24 hours before the appointment, and reminding them that to invite supports to the conference and them adding the notes appropriate systems.

    + FTC may be in person or virtual depending on the need of ACS and the family

    + Copies of FTC agreements should be completed by facilitator and provided to families.

    + The FTC facilitator is expected to be prepared to conduct the FTC by reviewing the case and discussing it with the case planner prior to the conference.

    + Complete note for FTC into CNNX and Promis systems.

    + Must schedule conferences based on the needs of the family while coordinating the attendance of Case Planners, ACS and any other stakeholder needing to be in attendance (i.e. school, therapist, Care Manager, family supports, etc.); Flexibility is a must as the conference might be held in the morning or evening as well as in an office, client’s home, school or other community location.

    + Successful completion of the Administration for Children Services (ACS) Family Team Conference Facilitator Training as well as the Family Connections training upon hire.

    + Provide training and support to the Referral Management team as needed.

    + Schedule trainings for all staff.

    + Update all FTC, and training logs.

    + Keep track of REA FASP for staff and send reminders emails about conferences to case planners and supervisors.

    + Keep track of all 30-45 day conferences and send reminder emails to case planners and supervisors.

    + Conduct all 30-45 day and service termination surveys to all family members 10 years and older.

    + Understanding the role of Preventive Services in the context of ACS’s Improved Outcomes for Children (IOC) initiative, including understanding of the need to assess for safety and risk throughout the life of the case.

    + Interfacing with ACS personnel as appropriate.

    + Working with the Prevention Program’s Quality Assurance person to understand the systems that have been put into place to aid good practice.

    + Meeting the expectations of the Provider Agency Management Review System (PAMS) and other auditing entities.

    + Completing Fidelity Check lists for all new cases opened.

    + Provide training to Secondary FTC Facilitator on all above required tasks as well as assist in training regarding process referrals.

    + Other duties as assigned.

    Requirements

    • Master’s level.

    • Bilingual preferred.

    Salary Description

    $61,206.60/year
