Job Summary and Qualifications

What you will do in this role:

You will assist and coordinate intake and pre-admission screening processes. You will document evidence of rehab admission criteria within established timeframe.

+ You will perform on-site and off-site clinical evaluations to assist Rehab Medical Director in establishing patient eligibility for admission.

+ You will provide information to patient/family/caregiver related to inpatient rehab and program features. You will communicate expectations and requirements for participation in the rehab program and conduct or coordinate tours of the rehab unit.

+ You will obtain information relating to patient resources and healthcare insurance benefits.

+ You will demonstrate clinical knowledge of rehab admission criteria to appropriately identify and qualify rehab patients.

What qualifications you will need:

+ Basic Cardiac Life Support must be obtained within 30 days of employment start date

+ Speech Therapist, or Occupational Therapy (OT), or Registered Nurse, or Licensed Physical Therapist

+ Masters Degree, or Associate Degree

+ 5 years of experience preferred
