    This hybrid role is primarily remote, with visits to the office once a month. The schedule is Monday – Friday, with the opportunity to move to a 4/10 schedule in the future. Candidates must reside near Portland, Oregon.

    Job Summary

    This position is responsible for daily operational oversight and supervision of Primary Care staff in order to ensure the highest level of compliance and quality care is provided. Responsibilities include operations oversight, patient advocacy, implementation and ongoing assessment of patient care needs, monitoring patient/family satisfaction, data collection, process improvement, staff and team productivity and compliance with regulatory requirements. This position supervises non-clinical staff.

    Essential Responsibilities

    Operations

     

    + Leads the daily work of the primary care interdisciplinary team, and act as a resource for staff.

    + Ensures daily activities are being assigned, monitored, scheduled, and coordinated across the team.

    + Ensures the highest quality of care is provided and is in compliance with established policies and procedures and federal, state, and local regulatory requirements by reviewing documentation, making visits with staff, reviewing care plans, participating in interdisciplinary care team (IDT) meetings, and reviewing patient satisfaction surveys.

    + Assesses hardware and software functionality and works with leadership, IS and/or vendors to resolve.

    + Leads, facilitates and participates in meetings or huddles, prepares agendas and distributes meeting minutes. Reviews meeting information with staff who are unable to attend.

    + Monitors and approves overtime, administrative and medical expenses to ensure adherence to budget.

    + Ensures the maintenance of clinical records by routinely auditing to assure accuracy, completeness and compliance with policies and procedures, regulatory requirements and standards including HIPAA.

    + Communicates with marketing personnel regarding patient issues that arise that may affect referral source relationships.

    + Performs field visits regularly to ensure staff quality care is provided and policies and procedures are followed. Holds staff accountable for correcting issues identified.

    + Consults with Primary Care Practice Administrator and Medical Director on medical issues and administrative responsibilities including QAPI and compliance.

     

    Planning

     

    + Participates in program management, including budgeting, strategic planning, utilization, and quality assessment/improvement (QAPI) activities.

    + Participates in development and implementation of policies and procedures.

    + Identifies areas for program and process improvement and makes recommendations to primary care management.

    + Analyzes staffing needs and provide recommendations to Primary Care Practice Administrator and Medical Director for decision making.

    + Creates and maintains staff schedule, reviews, and approves time off requests, and ensuring adequate staffing at all times.

    + Supervises team and recommends team direction and goals in alignment with the organizational mission, vision, and values.

    + Identifies work and staffing needs to meet work expectations; recruits and hires, using an equity, diversity, and inclusion lens.

    + Plans, organizes, schedules, and monitors work; ensures employees have information and resources to meet job expectations.

    + Leads the development, communication, and oversight of team and individual goals; ensures goals, expectations, and standards are clearly understood by staff.

    + Trains, supervises, motivates, and coaches employees; provides support toward employee development.

    + Incorporates guidance from CareOregon equity tools into people leadership, planning, operations, evaluation, and decision making.

    + Ensures team adheres to department and organizational standards, policies, and procedures.

    + Evaluates employee performance and provides regular feedback to support success; recognizes strong performance and addresses performance gaps and accountability (corrective action).

    + Performs supervisory tasks in collaboration with Human Resources as needed.

    Experience and/or Education

    Required

    + Minimum 4 years’ experience in a primary care setting

     

    Preferred

     

    + Minimum 1 year experience in a supervisory or lead position; lead roles should include regular assistance to a manager or supervisor in employee hiring, coaching, performance evaluation, and other supervisory tasks.

     

    Knowledge, Skills and Abilities Required

     

    Knowledge

     

    + Working knowledge and use of Trauma Informed language and principles, Motivational Interviewing principles, and community resources

    + Working knowledge of palliative care and end-of-life care principles
