In this vital role, you’ll function as part of an interdisciplinary team to ensure continuity of care for identified patient populations from admission through discharge. You’ll coordinate and guide each patient’s clinical plan—partnering with patients, families, physicians, and other inpatient care team members while adhering to regulatory and insurance requirements to support safe, timely discharges.
You’ll use your **critical thinking skills, nursing expertise, and compassion** to advocate for patients and support their recovery every step of the way—ensuring that care is effective, coordinated, and tailored to each individual’s needs.
This position is offered on a **day-shift schedule with no extra shifts outside of your set hours** , giving you the balance to grow your career while enjoying meaningful time with the people who matter most.

MINIMUM QUALIFICATION(S):
+ Two (2) years nursing experience
+ Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
+ AHA BLS prior to end of orientation period
PREFERRED QUALIFICATION(S):
+ BSN preferred.
+ Previous experience in case management, care coordination, outcomes management or discharge planning preferred.
+ Certification in case management from a nationally recognized organization preferred.
