    Volunteers of America Upstate New York's continuum of residential programs offer homeless & low-income individuals and families safe housing and support services. We provide long-term solutions to the problems of homelessness, addiction, incarceration, and poverty by helping the people we serve overcome obstacles and develop healthier, more self-sufficient lives.

     

    Case Manager – Family Shelter

    JOB SUMMARY:

    The Case Manager plays an integral role in providing comprehensive support and guidance to families experiencing homelessness who are placed in the shelter. This position involves assessing the unique needs of each family, developing individualized case plans and coordinating resources to help families achieve stability and ensure a successful transition to permanent housing.

    ESSENTIAL JOB FUNCTIONS:

    Assessment and Intake

     

    + Coordinate with Intake Coordinator to ensure all required paperwork and documentation has been completed and ensures files are complete and accurate

    + Works with families to complete required assessments and develop an appropriate independent living plan in accordance with regulations, community standards and VOA policy.

     

    Case Planning and Resource Coordination

     

    + Utilizing a person centered, trauma informed approach, works closely with clients to identify and connect families with relevant community resources and ensure all DHS requirements are met.

    + Met with clients on a weekly basis, or more as needed, to review goals and progress.

    + Establish and maintain positive relationships with community partners, government agencies, landlords and other stakeholders to enhance resources available to the shelter clients.

    + Connect families with relevant community resources in alignment with their independent living plan.

     

    Crisis Intervention and Advocacy

     

    + Provide immediate assistance and support during crisis situations

    + Serve as an advocate for families in the shelter, working to address barriers and promote self-sufficiency.

    + Assist in facilitating educational workshops and programs on relevant topics, such as parenting, job readiness and life skills.

     

    Documentation and Reporting

     

    + Maintain accurate records and case notes utilizing the Homeless Management Information System (HMIS) in accordance with VOA and community standards.

    + Prepare regular reports as required for internal and external stakeholders

     

    Client Services

     

    + Collaborate with other shelter staff to create a cohesive and supportive environment for families.

    + Participate in ongoing training to stay informed on best practices in homeless services.

    + Availability to work evenings as applicable to department environment.

    + Other duties as requested to benefit client and program.

    EDUCATION & QUALIFICATION REQUIREMENTS:

    + Associates degree in Human Services or Social work or equivalent

    + Preferred 2 years of case management experience with people experiencing homelessness.

    + Strong working knowledge of computer applications, including Excel and Word

    + Working knowledge of Department of Human Services (DHS)

    + Valid Drivers License with good driving record

    + Must obtain First Aid & CPR certification with ninety days of hire
