    SUMMARY OF POSITION:

    The Internal Medicine / Surgery Clinical Pharmacist plays an integral role in optimizing medication therapy and improving patient outcomes across general medical and surgical services. This position involves active participation in interdisciplinary patient care rounds, where the pharmacist provides evidence-based pharmacotherapeutic recommendations, ensures safe and effective medication use, and supports transitions of care.

    Key Responsibilities:

    + Deliver direct patient care through comprehensive medication management in collaboration with physicians, nurses, and other healthcare professionals

    + Participate in daily interdisciplinary rounds on assigned Med/Surg units

    + Monitor and adjust medication regimens based on clinical parameters, laboratory values, and evolving patient conditions

    + Precept and mentor pharmacy students and PGY1 residents as part of our training programs

    + Contribute to clinical research, quality improvement initiatives, and the development of protocols and guidelines that promote best practices in pharmacotherapy

     

    You will engage collaboratively with colleagues across various disciplines to implement innovative initiatives aimed at enhancing patient outcomes and fostering a culture of learning. Your responsibilities will include working in diverse clinical and operational settings within the department. We require a commitment to rotating day and evening shifts, as well as coverage for every third weekend and one major and one minor holiday per calendar year. Note: Day and evening shifts can include clinical and operational focused work, though preference for clinical shifts is given to clinical pharmacists.

    MINIMUM QUALIFICATIONS:

    + Bachelors of Science degree in Pharmacy or Doctorate Degree in Pharmacy required.

    + ASHP-Accredited PGY-1 Pharmacy residency or 3 years of relevant clinical pharmacy experience required.

    + Active and valid PA licensure or eligibility as a Pharmacist required.

    KNOWLEDGE, SKILLS, & ABILITIES:

    + Ability to prioritize and organize daily activities to meet patient care needs required.

    + Effective verbal communication skills, time management skills and ability to manage multiple priorities required.

    + Ability to analyze clinical data and perform calculations.

    + Computer and Microsoft Office skills required.

    + Extensive knowledge of pharmacotherapy required.

    + Attention to detail and critical thinking skills required.

    + Effective mentoring and precepting abilities required.
