    Job Details:

    The Clinical Data Abstractor is responsible for the activities of data abstraction, quality improvement, and outcomes reporting for the department, which are required for regulatory and NYS registries. The Clinical Data Abstractor will review the quality metrics and support the multidisciplinary clinical teams to review processes and make improvements as identified and needed. The data abstracted will be collected and submitted accurately and timely according to required registry and NYS deadlines. The goal is to promote positive patient outcomes through integration of performance improvement activities and measurement tools.

    Responsibilities:

    + Coordinates data and quality improvement activities for the department of Cardiology to actively support leadership and collaborate with the clinical team to drive departmental objectives, enhance patient outcomes, and foster a culture of continuous improvement.

    + Acts as a facilitator for performance improvement including coordinating efforts with key stakeholders such as physicians, nurses, ancillary staff, Information Technology and Revenue Cycle.

    + Creates and maintains key performance indicators that are aligned with national and state benchmarks to inform stakeholder decision making. Plans, timelines, and resource allocation strategies for improvement initiatives.

    + Monitors project progress, identify potential risks, and implement mitigation strategies to ensure timely completion of QI projects. Assist with the implementation of evidence-based practices and tools.

    + Oversees proper documentation of physicians and staff for the different requirements NYSDOH and specialty registries for the department. Provides real time feedback and education to the teams.

    + Abstracts clinical data from the EHR real time to ensure clinical team is providing standard of care and enters the various department required registries and regulatory agency sites.

    + Evaluate, review, and abstract clinical event data from electronic health records and other sources for quality improvement activities.

    + Maintains databases and audits charts as requested by State and National Specialty Registries.

    + Manages metadata for analysis and management.

    + Utilizes cardiology IT programs, NYS sites and analytics to collect volumes and quality.

    + Collaborates with the clinical team to analyze data and identify areas for opportunity of improvement and supports in implementation, action plans and follow up activities.

    + Analyze clinical data to detect trends and identify opportunities aimed at enhancing the quality and safety of patient care.

    + Prepares and maintains outcome metric presentation for the different sections of the department to be presented at hospital committees, and Department of Cardiology

    + Collaborates with clinical and administrative teams to ensure processes for patient care are streamlined and efficient to maximize productivity.

    + Participates on departmental and organizations committees and participates when required.

    + Participates in the registry and regulatory ongoing sessions and annual conferences. Communicates and implements required updates.

    + Collaborates in development of action plans to optimize workflows for care to ensure seamless access and care for patients.

    + Communicates data and quality outcomes internally to staff within the departments as well as through the quality and safety department, and quality committee of the board.

    + Performs other duties as assigned.

    Qualifications/Requirements:

    Experience: 1 to 3 years of nursing experience in a hospital setting with strong knowledge of Interventional Cardiology procedures, registry, and regulatory agencies required.

     

    Experience in data collection and analysis is strongly preferred.

     

    Knowledge and application of performance improvement methodologies is strongly preferred.

     

    Must be able to effectively interact and communicate with department heads, managers, and clinical staff.

     

    Education: Graduation from an accredited school of Nursing required; a bachelor’s degree or master's degree in nursing preferred.

     

    Licenses / Certifications: Valid license and current registration, issued by the New York State Education Department, as a Registered Nurse required.

     

    Other: Excellent intrapersonal and time management skills, thorough attention to detail and able to work with all levels of the department, keeping abstracted data confidential.
