    Job Summary

    Patient Blood Management ( PBM ) is a patient-centered and evidence-based approach applied at the system-level to improve patient outcomes by managing and preserving a patient’s own blood. 21% of patients nationally are iron deficient, defined as ferritin < 30 ng/mL. The nurse practitioner (NP) or physician’s assistant (PA) in this position will support UUH’s PBM program by providing care within the Hematology Clinic at Sugar House Health Center, primarily to patients identified as being iron deficient. This is an expansion of the Hematology Clinic at Sugar House Health Center, which will focus specifically on the diagnosis, treatment and/or management of iron deficiency with or without anemia.

     

    This clinic is expected to consist mainly of virtual visits being the primary examination mode. When the occasional on-site clinic visit is needed, this will occur at the Sugar House Health Center, but as the position evolves, these rare on-site clinic visits could occur at any UUH or partner locations. This fully benefitted role within the UUH School of Medicine, Department of Internal Medicine, Division of Hematology, is full time, M-F, 8:00 AM to 5:00 PM with flexibility to work 32 to 40 hours per week (0.80-1.0 FTE ) based on the candidate’s preference.

    Responsibilities

    Essential Functions

    1. Conducts comprehensive assessments, diagnose, order, and interpret diagnostic tests.

    2. Assists and directs support staff in the management of a referral queue and clinic flow.

    3. Initiates or recommends pharmacologic and non-pharmacologic therapies within scope of practice.

    4. Performs selected procedures for therapeutic and/or diagnostic reasons within scope of validated competence.

    5. Provides patient and family education.

    6. Meets with patients, families, and significant others about health and illness, with the goal of promoting ongoing health maintenance.

    7. Evaluates, plans, implements, and re-evaluates the care of individuals requiring emergency measures.

    8. Attends educational in-services as appropriate. Participates in professional activities and organizations to maintain knowledge of current trends, practices, and developments.

    9. Complies with medical record policies regarding the complete and accurate documentation of patient care initiatives.

    10. Participation in quality, patient experience, and efficiency improvement activities.

    11. Actively supports the Division and Program initiatives and goals.

    12. Works with Directors of Advanced Practice and Classical Hematology Medical Director to formulate a plan for professional development.

    13. Gains an understanding of our culture, operational strategy, market, and patient populations.

    14. Respects diverse views and approaches, and contributes in maintaining an environment of professionalism, respect, civility, and acceptance toward all employees, patients and visitors.

    15. Performs other similar and related duties as required or directed.

    Minimum Qualifications

    Required

    Licensure to practice as a Nurse Practitioner in the State of Utah.

     

    Preferred

     

    Certification in ACLS / BLS / NRP / ATLS may be required by your department or by privileging criteria.

     

    Utah State prescriptive privileges.

     

    Licensure is obtained by completing a Master’s degree in Nursing or equivalency (the only equivalency to the Master’s degree is current licensure as a Nurse Practitioner) and a National Certification which requires the successful completion of a nationally accredited examination.

     

    UHPPM 1-56 requires that newly hired non-physician providers apply for credentialing and privileging at the time of hire.

     

    This position is patient-sensitive and must fulfill all associated requirements.
