The Department of Surgery at Dartmouth Health seeks an experienced Advanced Practice Provider to support our Colorectal Surgery division. The Colorectal Surgery (CRS) APP will play a critical role in the division’s expansion into a more robust and structured urgent referral program coordinator and clinical APP. This clinical role will focus on early access for urgent patients in need of specialist and seamless continuity of high-quality care throughout the entire continuum of disease both complex benign and malignant disease management—from diagnosis through post-surgical surveillance.
The CRS APP will collaborate closely with the multidisciplinary colorectal cancer team, including colorectal surgeons, gastroenterology, medical and radiation oncologists, nursing, social work, and other allied health providers when indicated, to reduce clinical backlog, enhance operational workflows, and implement essential infrastructure for care coordination and quality tracking.
Key Responsibilities:
* Provide direct patient care in the outpatient setting at both DHMC and Valley Regional Hospitals with a focus on anorectal disease management and long-term cancer surveillance
* Conduct new patient consultations, follow-ups, and procedural visits under the supervision of attending colorectal surgeons.
* Manage and complete patient documentation, orders, and treatment plans in compliance with institutional standards.
* Serve as a point-of-contact for patients navigating complex care pathways, including surgery, neoadjuvant/adjuvant therapy, and watch-and-wait protocols.
* Collaborate with existing clinical team members to balance workloads and reduce risk of provider burnout, offering mentoring and peer support to junior APPs as appropriate.
* Oversee a new tracking system to monitor individuals throughout their oncology journey (staging, initial consult, treatment planning, pre-surgical therapy, surgery, and surveillance) facilitating timely scheduling of treatment modalities and monitoring any complications of those non-surgical treatments.
* Monitor adherence to clinical protocols, including "watch and wait" rectal cancer treatment paradigms, and flag deviations that may compromise outcomes or safety (including delays in care, missed follow-ups or lapses in communication across care teams).
* Coordinate with the clinical research coordinator to maintain clinical database.
* Dedicated triage management and review of urgent referrals, both benign and malignant, for completeness, urgency, and complexity, ensuring timely and assist secretaries in appropriate scheduling.
* Coordinate with referring providers, radiology, pathology, and scheduling teams to ensure all necessary diagnostics are completed pre-consult.
* Serve as a clinical point of contact to improve referral quality, reduce administrative burden on surgical attendings, and standardize intake protocols.
