As a Program Coordinator, you will play a pivotal role in mentoring residential leadership, guiding direct support professionals, and ensuring that individuals we serve live fulfilling, safe, and independent lives integrated into their communities. OLV Human Services operates a total of 8 IRAs (Individualized Residential Alternatives) across Western New York that are staffed with Direct Support Professionals (DSP). The 8 IRAs create 2 different departments: Northtowns (Amherst x2 houses, Buffalo, Tonawanda, Cheektowaga) & Southtowns (Lackawanna x3 houses).

FULL JOB DESCRIPTION
Job Summary:
The Program Coordinator is responsible for coaching and mentoring the residential management team and direct support professionals in their department and establishing a culture that supports individuals with developmental disabilities to be as independent and productive as possible in their daily life and to be meaningfully integrated into the community, including ongoing engagement with their family.
Essential Job Duties:
- Responsible for the overall operation, either directly or through delegation, of all the residential sites in their assigned department
- Establishes the tone, expectations and culture of accountability for the departments and acts as a leader for the division.
- Responsible to ensure that assigned sites are operating in accordance with all DD Division and OLV Human Services agency policies and all OPWDD regulations.
- Oversees that assigned sites operate within established fiscal parameters.
- Oversees that assigned sites are maintained in a welcoming/safe manner.
- Effectively resolves problems related to the operation of and provision of services within assigned sites and/or seeks appropriate assistance from the Director of Residential Services.
- Attends all required internal trainings to maintain competence in position and attends external trainings as directed or per request, for ongoing professional growth and development.
- Develops, in conjunction with the team, a staff action plan and safeguarding document (IPOP) for each individual served, which assists all residential staff in providing appropriate supervision training and support for the individuals.
- Leads the Site Team in weekly discussion to ensure that all medical appointments are completed as scheduled and that all necessary follow up is completed in a thorough and timely manner.
- Maintains complete and accurate program records that verify daily provision of serves for each individual, ensuring that DSPs are completing meaningful documentation records (Including but not limited to: Life Plans, LCED’s, meal intake, medical monitoring, medication administration, staff action program data/goals, Therap notes, behavior notes, incident reports, etc.. ).
- Assists to sign off and review staff action plans and monthly summaries as needed.
- Completes or ensures completion of all billing information for the Business office.
- Acts as primary liaison with school/day program providers, to ensure consistent and integrated day services.
- Completes routine compliance and Quality of Life Reviews.
- Regularly communicates with parents/guardians/advocates regarding the needs and status of the individual being served.
- Regularly communicates with the Care Coordinator for each individual regarding need for and/or appropriateness of services and benefits
- Supervises, Trains, Mentors, Coaches and Supports the management team in the effective supervision/coaching/mentoring of the DSPs.
- Ensures that management team are maintaining appropriate DSP staffing levels for the department and that overtime hours are warranted.
- Assists, as needed, with recruiting, interviewing, hiring, and training of staff to maintain a high quality and effective team.
- Oversees that the management team is appropriately collaborating with Human Resources relative to disciplinary actions for DSPs, consistent with the timeframes and guidelines set forth in the union contract.
- Takes the lead and oversees Therap use by all staff in their department, to ensure that a comprehensive medical and clinical record is maintained.
- Oversees appropriate security, handling and disbursement of in-house Personal Allowance funds, assisting with this process as needed.
- Reviews and signs off on all minor incident reports and completes required documentation and notifications for all reportable, serious reportable and abuse allegation incidents.
- Completes all necessary team meetings and required paperwork.
- Processing of all new admissions to assigned sites.
- Participates on division and/or agency committees, as assigned.
- Complies with all federal and state regulations and agency policies pertaining to HIPAA, false claims, whistleblower policies and all others.
- Performs other duties and assumes other responsibilities as may be assigned by the Director of Residential Services.
- Any other duty as assigned by your Supervisor.

Qualifications:
Minimum Education Required:
- Bachelor’s Degree in Human Services or related field
Minimum Experience Required:
- One (1) year prior experience working with individuals with disabilities
- One (1) year supervisory experience is preferred
License/Registration/ Certifications Required:
-Valid NYS Driver’s License

Physical Requirements:
The overall nature of the position is sedentary requiring little physical effort with occasional light physical exertion required. There is little, if any, exposure to environmental conditions.
• The constant physical demands of the position are standing, walking, talking, and hearing.
• The frequent physical demands of the position are sitting, climbing, repetitive motions, and eye/hand/foot coordination.
• The occasional physical demands of the position are lifting, carrying, pushing, pulling, balancing, stooping, crouching, reaching, handling, grasping, and feeling.
