    Job Summary

    Coordinates admissions process to Inpatient Rehabilitation Center (IRC) in conjunction with clinical practitioners, patients/caregivers, referral sources, payers, and Financial Counseling. The PAS (Pre-Admission Screen) Care Coordinator serves as case management leader of the complex admission/discharge planning process and a valued member of the healthcare team. The PAS Care Coordinator collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive/complex/ safe discharge plans. The PAS Care Coordinator is also responsible for facilitating coordination of care. The PAS Care Coordinator along with the other members of the healthcare team accepts responsibility, authority and accountability for management of care. This shall be in accordance with the current policies and procedures, mission, vision, and values of Novant Health. Serves as a resource for discharge planning and care coordination for medically complex cases. Maintains current knowledge of available community resources/post-acute care options.

    Qualifications

    + Education: Graduate of an accredited professional program resulting in licensure as RN, MSW, PT, OT or SLP, required. 4 Year / Bachelors Degree, preferred.

    + Experience:

    + Two years Clinical and healthcare experience, required.

    + Four years Experience in a rehabilitation setting, preferred.

    + Licensure/Certification:

    + Current licensure: RN, MSW, PT, OT or SLP, required.

    + CIRS, CRRN, CCM, CRC or equivalent certification, preferred.

    + Additional Skills (required):

    + Knowledge/competence of Microsoft office products and keyboarding

    + Ability to successfully complete generic and department-specific skills validation and competency testing. Professional and personal effectiveness in establishing relationships with referral sources, payors and patients/caregivers.

    + Additional Skills (preferred): Centers for Medicare and Medicaid Services (CMS) regulations for IRF-PPS.
