    Baystate Health, a nationally recognized leader in healthcare quality and safety, and home to **Baystate Franklin Medical Center** is looking for a **Registered Nurse (RN) Case Manager** to join our Medical Surgical Unit in **Greenfield, MA** !

     

    **Location:** Baystate Franklin Medical Center

    **Schedule:** 32 hours, 7a-3p, every third weekend (once per month)

    Job Basics:

    + **Coordinate and manage patient-centered care plans** from admission through discharge, including clinical assessments, discharge planning, and care transitions across the continuum.

    + **Collaborate with physicians, nurses, and community partners** to ensure appropriate resource utilization, continuity of care, and timely progression toward discharge.

    + **Apply evidence-based criteria** to evaluate medical necessity and level of care; communicate with insurance providers to support authorization and optimize reimbursement.

    + **Identify high-risk patients and reduce readmissions** through proactive planning, clear communication with patients and families, accurate documentation, and ongoing professional development.
