    Description

    As a Senor Licensed Community Clinician, you will provide home and community-based assessments, treatment planning, and individual and family therapy with children and their caregivers. You will provide clinical and therapeutic services to children (up to age 21) and their caregivers through this Medicaid- funded program. You will provide services to children and their families in the setting most beneficial to them including home and community--based, office--based, telehealth, or a combination thereof, as agreed upon in the child's treatment plan.

     

    You will report to the Clinical Supervisor/ Clinical Director. You will work a hybrid schedule, combining on-site and remote work, with up to 25% community travel and client home visits.

    Responsibilities:

    + Provide home and community-based assessment, treatment planning, and individual and family therapy to children and their caregivers.

    + Provide clinical and therapeutic services within a flexible model/ setting (i.e. home and community--based, office--based, telehealth, or a combination thereof, as agreed upon in the child's treatment plan).

    + Complete an assessment of needs and recommendations for further medically necessary services, such as rehabilitation services. Services are delivered in a trauma informed, culturally and linguistically competent manner.

    + Work with family members, advocates and other salient parties to identify goals of treatment. Provide observations and information to collateral contacts to address and achieve treatment goals.

    + Complete all required documentation for submission to the supervisor within the required timeframes.

    + Provide clinical supervision to program managers and direct-line staff

    + Provide clinical support to community service teams, including case consultation and treatment plan support

    + Collaborate with resources, both internal and external to the agency to assist in the process for the provision of Evidenced-based Practices under State Plan services.

    Requirements and Education:

    + Masters degree in Social Work, Counseling, Psychology or related field required

    + 1+ year of related experience required.

    + NYS Professional Licensure LMFT, LMSW, LMHC, LMHC- D, LCSW,and LCAT required.

    + NYS drivers license with a reliable vehicle required.
