    Job Description:

    The Transition Guide supports the transition process for adolescent and young adult patients seen at Intermountain Children’s Health clinics as they transfer care to adult providers. As part of a centralized transition team, they will work with patients, clinic staff, and administration to provide a quality patient experience.

    Posting Specifics

    + **Pay Rate:** Based on experience

    + **Shift Details** **:** Full-time (40 hrs/wk) **Monday - Friday. Typical hours are anywhere between 8:00 am – 5:00 pm (we will sometimes be involved in community events that occur during the evening hours)**

    + **Department** : Children's Health Admin - Primary Children's Hospital

    + **Additional** **Details:** This is a hybrid position. **Most of the work is done remotely. Incumbent needs to have high internal motivation to drive work, but occasional (** **as frequent as once a week** **), in person work is involved.** **Experience working with adolescents / young adults or the disability community is highly preferred.**

    Essential** **Functions

    + Develop transition processes with clinic staff to support patient and family transition needs.

    + Track metrics and outcomes for the program.

    + Use motivational interviewing and health coaching skills to best support skill building with youth and their families.

    + Work independently as directed by team leader and clinic leads.

    + Teach and empower youth to develop skills that will help them transition to adult care successfully.

    Required** **Qualifications

    + Experience in a role requiring effective organization skills and follow through.

    + Ability to communicate with others in a clear, understandable, and professional manner.

    + Strong interpersonal skills.

    + Ability to work with personnel at all levels to gather information and handle complex issues within an often-sensitive political environment.

    + Experience working collaboratively in a team environment.

    + Experience using word processing, spreadsheet, database, internet and e-mail, and scheduling applications.

    Preferred Qualifications

    + Associate degree

    + Experience in healthcare or hospital setting

    + Understanding of medical terminology.

    + Experience in patient care management, transition/discharge planning, medical assistance, or patient care coordination.

    + Ongoing need for employee to see and read information, labels, documents, monitors, identify equipment and supplies, and be able to assess customer needs.

    + Frequent interactions with providers, colleagues, customers, patients/clients and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.

    + Manual dexterity of hands and fingers to manipulate complex and delicate supplies and equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc.

    The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

     

    $25.02 - $39.41
