    Job Summary:

    The MDS Assistant supports the MDS Coordinator in the accurate and timely completion of the Minimum Data Set (MDS) assessments and related documentation. This position helps ensure regulatory compliance, supports care planning, and contributes to quality resident care and reimbursement accuracy.

    Key Responsibilities:

    + Assist in gathering and organizing clinical documentation and data for MDS assessments.

    + Collaborate with interdisciplinary team members to ensure complete and accurate resident information.

    + Enter data into MDS software systems and electronic medical records.

    + Help maintain MDS schedules and track due dates for assessments and care plans.

    + Follow up on missing or incomplete documentation from clinical staff.

    + Support the preparation and submission of MDS reports to CMS (Centers for Medicare & Medicaid Services) and other regulatory bodies.

    + Maintain up-to-date knowledge of MDS 3.0 guidelines, RAI Manual requirements, and state-specific regulations.

    + Participate in quality assurance and audit activities as assigned.

    + Maintain confidentiality and comply with HIPAA and facility policies.

    + Perform other administrative or clinical support duties as directed by the MDS Coordinator or Director of Nursing.

    Qualifications:

    + LPN or GPN certification

    + Prior experience in a long-term care or healthcare setting preferred.

    + Working knowledge of MDS 3.0 and RAI process is a plus.

    + Proficient in Microsoft Office and PointClickCare.

    + Strong organizational skills and attention to detail.

    + Ability to work collaboratively and communicate effectively with clinical teams.

    + Understanding of medical terminology and documentation standards.

    Working Conditions:

    + Office setting within a healthcare facility.

    + May involve occasional interaction with residents and clinical staff.

    + Must be able to sit or stand for extended periods and use a computer daily.
