About this role:
With guidance and support from the Director of Support and Stabilization, the Youth Mentor provides supportive community mentoring services to youth ages 5 to 21, residing in residential placements, foster care placements and biological homes. The Youth Mentor assists the program participant in planning and implementing skills that lead to the development of independence, safety, self-growth and more! The Youth Mentor promotes individuals' growth in their areas of interest, decision making, community engagement, and personal accomplishments.
Schedule: Sunday through Thursday, 9 am - 5 pm with needed flexibility
Qualifications:
+ Bachelor’s Degree in the Human Services field, preferred.
+ 5 years’ experience in a related field, required.
+ Driver's License and reliable vehicle.
+ A therapeutic and empathetic awareness to individuals with exposure to intense trauma.
+ Strong Interpersonal and communication skills.
+ Commitment to community-based supports to individuals.
+ Able to meet required competency level in Microsoft Office software.
+ Knowledge of community resources available to assist participants in achieving their goals.
Physical Requirements:
+ Extensive travel required
+ Ability to sit for extended periods of time
+ Ability to climb stairs
+ Ability to make in-home visits to children and their families
+ Ability to use a phone and computer for extended periods of time
Responsibilities:
+ Provide one on one support to program youth on a weekly or more basis.
+ Provide transportation to youth as needed.
+ Collaborate with Support and Stabilization and IFC team members to ensure adequate services.
+ Supports youth and adolescents in achieving their personal development, home-based, and community-based skills, economic stability, and development of positive social networks.
+ Learn and adhere to the Kennedy-Donovan Center and the Department of Children and Families (DCF) philosophy and mission statements.
+ Develop a respectful and trusting rapport with youth.
+ Participate in the development of treatment plans.
+ Ensuring visits are goal centered based on youth's needs and DCF's recommendations.
+ Provide advocacy for individuals as needed.
+ Participate in IEP and care coordination meetings as needed.
+ Collaborate with internal and external community resources and providers to ensure safe and appropriate placement services.
+ Participate in DCF meetings as needed, weekly Support and Stabilization group and weekly division meetings.
+ Comply with all KDC and DCF documentation requirements.
+ Assist with identifying age-appropriate resources.
+ Coordinate and support enrichment activities such as arts, drama, sports, recreation, and engagement with animals.
+ Assisting and encouraging adolescents and young adults with building positive social networks, managing finances, promoting independence and other responsibilities of adulthood.
+ Assessing youths’ skills for self-sufficiency and community-based living using the Youth Readiness Assessment Tool.
+ Provide organizational assistance if indicated in treatment planning.
+ Perform other job-related responsibilities as assigned.
