Offer Based on Years of Experience Salary: $72,000 to $77,000 per year DOE This is a fully Work From Home opportunity SHIFT: Multiple Shifts available weekday and weekend all Full Time Previous Home Health or Hospice experience preferred HomeCare Home Base experiece preferred What You Need to Know Job Duties: Responsible for the quality of home health patient Care Plans in collaboration with assessing clinician and coding coordinator. Utilize referral documents, orders, comprehensive assessments, and OASIS documentation to edit/build a detailed and individualized POC that demonstrates home health eligibility and skilled need. Maintain up to date status information on any assigned cases that are unable to be completed due to outstanding information. Notify leadership of problematic trends identified during case reviews. Maintain professional and technical knowledge by attending educational workshops and reviewing professional publications. Assist with other chart audits/tasks as assigned. If you meet these qualifications, we want to meet you! An associate degree in nursing with current unencumbered licensure to practice nursing in the state of residence required. Bachelor’s degree preferred. Minimum 1 year experience in the health care required. Home health experience preferred. 1 year HCHB experience strongly preferred Oasis Certification will be requried for this role.
