    Territory:** **Team 2 encompasses Berwyn, Broadview, Riverside, but also Chicago.

    _Position Purpose:_

    The Palliative Care RN is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation.

    _What You Will Do:_

    + Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation

    + Motivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy

    + Provides statistical/budgetary data to appropriate Palliative care program

    + Works collaboratively with all departments to ensure execution of quality delivery of services

    _Minimum Qualifications:_

    + Graduate of an approved nursing education program.

    + Current Registered Nurse licensure in the state of practice.

    + CPR certification

    + Hospice/Palliative Care certification preferred.

    + Home care experience, strong clinical knowledge base, Hospice experience, computer skills, strong interpersonal and organizational skills,

    + Palliative Care experience, demonstrate ability to use problem solving, critical thinking and priority setting skills

    + Must be able to work independently and on a team

    + Ability to perform diversified duties with time limitations

    + Ability to develop, coordinate and evaluate programs in Palliative care and end-of-life care
