    Coordinates program activities at the various St. Luke’s campuses in accordance with grant/program guidelines and contracts. Outcomes include measurable results in placement of grant participants in productive employment at St. Luke’s/Community, high school graduation, and enrollment in post-secondary education.

    JOB DUTIES AND RESPONSIBILITIES:

    + Plans and implements the Observational and Work Experience Programs for high school students in a metrics-driven environment.

    + Collaborates with local school districts and community partners to recruit Adolescent Career Mentoring Programs participants.

    + Assesses youth for work readiness skills, strengths, aptitudes and any potential limitations/barriers to employment and post-secondary education.

    + Assesses and determines eligibility for services, according to government and local program guidelines.

    + Collaborates with St. Luke’s Volunteer Services and Human Resources department on the onboarding of Adolescent Career Mentoring Program participants.

    + Visits students at job sites at least once bi-weekly.

    + Provides one-on-one career coaching to increase employability skills, develop professional career goals and identify career ladders for program participants.

    + Supervises and mentors program participants.

    + Provides wrap-around case management services to participants to address the social determinants of health, and document detailed case notes.

    + Prepares and submits reports to the Network Director of Community Workforce Development and/or funding agency.

    + Tracks student data across multiple softwares and platforms.

    + Assists with the management of grant funds, including compiling and preparing documents for invoicing, in collaboration with Network Director and Grants and Quality Improvement Project Manager.

    + Monitor students’ timecards to ensure compliance with grant requirements.

    + Prepares, maintains, and updates participants’ files in accordance with grant/program guidelines and contracts.

    + Plans, implements and facilitates professional development sessions to increase job keeping and job seeking skills for program participants. Program sessions may include resume writing, interview workshops and job applications.

    + Conducts preceptor enrollment and training.

    + Organizes and hosts parent/student meetings.

    + Assists youth in completing college, financial aid, and scholarship applications to enroll in post-secondary education, and monitor youth academic performance to ensure completion of college/training program.

    + Acts as an advocate and liaison between the participants/families and community agencies (i.e. schools, social service agencies, health programs, and employers) in the Lehigh Valley Area.

    + Develops and maintain relationships with prospective employers in the Lehigh Valley as well as with St. Luke’s Departments/Units and Human Resources.

    + Attends industry meetings/events to gain a detailed understanding of the sector and employment opportunities in the Lehigh Valley.

    + Develop effective working relationships within the hospital, community and program partners.

    + Other related duties as assigned.

    PHYSICAL AND SENSORY REQUIREMENTS:

    Sitting for up to four hours per day, up to two hours at a time. Standing for up to six hours per day; three hours at a time. Walk six hours per day; 20 minutes at a time. Frequently use hands and fingers. Consistently lift, carry and push objects up to 25 lbs. frequently stoop and bend. Frequently reach above shoulder level. Must be able to perceive attributes of an object through touch. Must be able to hear as it relates to normal conversation, and high and low frequencies. Must be able to see as it relates to general, near, far, color and peripheral vision.

    EDUCATION:

    A Bachelor ’s degree preferred, preferably in related field (Education, HealthCare, Public Health).

    TRAINING AND EXPERIENCE:

    Experience in community-based outreach and working with diverse populations preferred.
