Job Title: ShabazDepartment: LTC Location: Park Ridge Living Center Hours Per Week: 37.5Schedule: Evenings 3pm - 11:30 pm Monday - Friday, every other weekend and holiday Sign-On Bonus: up to $4500 SUMMARY:

As a Shahbaz, you are responsible for assisting our residents with a wide range of their daily needs and undertakings. In addition to direct patient care, they are responsible for meal preparation, house cleaning and various recreational activities.

RESPONSIBILITIES:

+ Patient Care. Provide and assist with all aspects of daily living activities, including personal hygiene, dressing and comfort care; maintain a safe and attractive environment at all times

+ Meal Preparation. Prepare meals based on Food Production Sheet and create alternative choices to meet patients’ preference and nutritional needs; ensure all kitchen logs are in compliance with NYS Department of Health regulations; maintain a safe and sanitary environment

+ Housekeeping. Monitor housekeeping and laundry logs daily; coordinate cleaning schedules

+ Activity Management. Follow Recreation Community Calendar for daily plan, and check with patients for any activity preference; coordinate with the team and implement morning and afternoon activities daily; facilitate community outings; document all activities in Caretracker

REQUIRED QUALIFICATIONS:

+ Registered Certified Nursing Assistant (CNA) required

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
