    Job Title: Occupational Therapist

     

    Department: PRLC Occupational TherapyLocation: Park Ridge Living CenterHours Per Week: 40Schedule: Monday - Friday, Days 8am to 430pm

    SUMMARY:

    As an Occupational Therapist, your strong attention to detail and excellent evaluation skills are crucial to successfully restoring or improving each patient’s optimal level of function, independence and quality of life.

     

    Park Ridge Living Center, which houses Wegman Family Cottages, has achieved and maintained a five-star rating from the Centers for Medicare and Medicaid Services (CMS) since 2013. Only 10% of nursing homes in the country attain this five-star rating. Conveniently located on the Unity Hospital Campus, the Wegman Family Cottages have been serving elders in our community since 2010. Our mission is to provide quality care, independence, and a positive living experience for patients and elders.

    RESPONSIBILITIES:

    + Evaluation & Development - Perform a complete initial evaluation of referred patients using evaluation protocols, standardized tests and clinical observations; develop and implement treatment plans, including short- and long-term goals and methods to achieve them; adjust treatment plan and goals as needed .

    + Documentation & Communication - Maintain complete documentation of patient evaluation, treatment, progress and discharge status; counsel and teach patients and families; coordinate care plans with other involved health professionals and discuss changes in treatment programs with MDs .

    + Employee Development - Assist with the supervision and guidance of new personnel, assistants, students and volunteers as assigned; develop and conduct in-service training programs .

    REQUIRED QUALIFICATIONS:

    + Degree in Occupational Therapy, bachelors, masters or doctorate.

    + Current NYS Occupational Therapist License or limited permit as a Registered Occupational Therapist.

    PREFERRED QUALIFICATIONS :

    + Three (3) to six (6) months of previous experience preferred .

    + Compassionate , kind and patient focused .

    + Excellent communication and customer service skills .

    + For ElderONE Employment: 1 year experience with frail elderly population preferred .

    EDUCATION:

    BS: Occupational Therapy (Required)

    LICENSES / CERTIFICATIONS:

    BLS - Basic Life Support - American Heart Association (AHA), OT - Occupational Therapist - New York State Education Department (NYSED)

    PHYSICAL REQUIREMENTS:

    M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

    PAY RANGE:

    $68,500.00 - $97,760.00

    The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
