Description:
The Case Manager LMSW is a professional who collaborates with physicians, health care team members and patient/families to coordinate care for specific patient populations across the continuum and between care settings during an episode of illness.

**Shift** : Varies
**Position Purpose** : Case Manager LMSW I to work in the Case Management Department at Grand Haven Hospital.
What you will need:
+ Masters degree in social work required
+ Prior employment experience in a related area preferred
+ State of Michigan, social work license required
What you will do:
+ Identify, coordinate communicate and participate in discharge planning with patients and a multidisciplinary team
+ Recognize and monitor potential barriers to safe discharge planning and intervene as appropriate
+ Navigate community resources as well as serve as a reference and liaison for community resources and service.
+ Perform conflict resolution and crisis intervention when and where appropriate
+ Accurately document interventions, assessments, and discharge planning in the electronic medical record
