Wage:$25.00/hour
Job Status: This position is part-time, 20 hours per week. For the right applicant, this position could be full-time, 40 hours per week and serve customers across Park, Meagher & Gallatin Counties.
HRDC also has a "Wellness Hours Policy" which encourages full-time staff to use up to 4 paid hours weekly to work on their wellness.
General Summary:The Senior Case Manager plays a crucial role in providing direct assistance to older adults. This position is dedicated to connecting individuals and families with a wide array of community resources, including healthcare services, transportation options, housing assistance, and social programs. The Senior Case Manager will assist customers in navigating the complex systems of community services available to seniors.
In addition to resource coordination, this role emphasizes the importance of building and maintaining positive, trusting relationships with individuals and their families. Collaboration is key in this role; the Case Manager will work closely with other staff members across the agency. This includes participating in case conferences, sharing insights on best practices, and contributing to program development initiatives that aim to enhance the overall service experience for older adults.
Primary Job Duties and Responsibilities:
1. Customer Support/Assessment of Need (65%)
Working with customers to determine appropriate program placements and referrals; informing customers of available services and resources; coordinating and supporting customers’ access to services and benefits.
2. Program Eligibility (25%)
Works with Compliance Specialists and program manager to determine program eligibility and benefit level by identifying and requesting proper documentation; accurately calculates household income and benefit levels; determines household status and processes household changes as required by various funding sources. In conjunction with the Compliance team, maintain customer files, case notes, and appropriate documentation systems. Enter program data efficiently and accurately into required databases.
3. Teamwork (5%)
This role actively engages in collaboration with all agency programs, working diligently to maintain and enhance best practice standards in case management. Through effective teamwork and communication, it ensures that the highest levels of care and support are consistently provided to those in need.
4. Training and Development (5%)
Actively participates in continuous training sessions that focus on industry best practices, current regulatory standards, and the latest organizational policies to enhance skills and knowledge.

Travel:
+ This position primarily serves customers in Gallatin County, but may also support Park and Meagher counties.
+ This position requires the incumbent to occasionally transport customers to appointments using agency vehicles.
Language Skills:
+ Native/Bilingual Spanish-speaking preferred
+ Ability to read, interpret, and write routine documents and correspondence in English, Spanish preferred
+ Ability to analyze and interpret complex and sensitive information; persuasive speaking abilities
+ Ability to effectively disseminate information about HRDC and community programs.
Mathematical Skills:
+ Basic math skills, including the ability to calculate annual household income
Reasoning Ability:
+ Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists
+ Ability to interpret a variety of program manuals, guidelines, and instructions furnished in written, oral, diagram, or schedule form
+ Ability to define problems, collect data, establish facts, and draw valid conclusions
Confidentiality:
+ This position requires regular handling of sensitive and confidential client and agency information. Unauthorized disclosure could harm client interests and the organization, and may also violate state and federal privacy laws. This position will have full access to reports, records, and personnel files, and must exercise the highest level of discretion, judgment, and confidentiality at all times.
Computer Skills:
+ Proficient in navigating spreadsheets, multiple databases, email, calendar
+ Willingness to learn new software systems
+ Comfortable with Microsoft and Google operating systems
Drivers License:
+ Valid State Driver License required
+ Reliable vehicle and appropriate liability insurance are required
Licenses or Certifications that would be useful but are not required:
+ CPR and First Aid, Trauma Informed Care, Mental Health First Aid

+ Ability to effectively work and communicate positively with all socio-economic groups
+ Ability to effectively perform duties with a thorough knowledge of agency goals, objectives, projects and services
+ Enjoys the balance of working directly with people and completing administrative tasks related to customers files.

+ Ability to demonstrate a strong commitment and patience in helping people succeed in innovative training programs, re-orient their lives and enable them to make a difference in their community
+ Demonstrate knowledge of community resources and partner agencies
+ Ability to effectively establish and maintain positive working relationships with other social service providers, organizations, program participants, and staff

+ The incumbent must be able to perform multiple tasks simultaneously, must be able to work under time and/or deadline pressures.
Education and Experience:
+ Bachelor’s degree in Psychology/Social Services/Education or related field, from a four-year college or university preferred, or comparable life/work experience.
Supervision Received:
+ This position operates under general supervision.
+ The incumbent’s work is primarily assigned by daily workflow and operations.
+ The incumbent is authorized to make decisions regarding the timing and sequence of work, and the work is performed in accordance with established policies, procedures, and applicable laws, rules, and regulations governing the tasks being performed.
Supervision of Others:
+ None.
Financial Responsibility:
+ Responsible for data entry for payable claims with proper coding according to eligibility requirements and communication with our compliance team.
Personal Contacts:
The incumbent is responsible for the following personal contacts (either in oral or written form) when performing the Duties and Responsibilities of the Position:
+ Daily interaction with the public or consumers
+ Interacts with program managers, executives, HRDC boards, and/or committees several times a year
+ Interacts with representatives of other service providers on a weekly or more frequent basis
+ Interacts with funding agencies to provide information or exchange facts annually
+ Interacts with governmental compliance or auditors to provide information or exchange facts annually
Working Conditions:
+ Works in a temperature-controlled office environment.
+ Weekly or more frequent operation of a motor vehicle under all weather conditions.
+ Weekly or more frequent exposure to customers with unfavorable body odor.
+ Weekly or more frequent exposure to customers in crisis, which can manifest in loud outbursts, anger, and threats of harm to self or others.
