    Job Title: Case Manager

    MUST HAVE

    + Active LCSW license or RN license.

    + BLS certification through the American Heart Association.

    + 2+ years of hospital case management/care coordination experience.

    + Proficiency in Epic charting system.

    + Strong knowledge of discharge planning, care coordination, and community-based services.

    + Familiarity with major disease processes including CHF, COPD, dementia, diabetes, and psychiatric disorders.

    + Previous charge experience preferred but not required.

    + Experience in teaching or community hospitals, trauma centers preferred but not required.

    + Experience with protective services, hospice, and long-term care coordination preferred but not required.

    + Excellent communication and interpersonal skills.

    Job Description

    We are seeking a dedicated and experienced Care Manager, either a Licensed Clinical Social Worker (LCSW) or Registered Nurse (RN), to support hospital care management and discharge planning. This role is critical in coordinating patient care, facilitating safe discharges, and ensuring continuity of services across care settings.

    Responsibilities

    + Conduct initial evaluations and develop individualized plans of care.

    + Coordinate discharge planning, including arranging home care, equipment, and follow-up services.

    + Collaborate with internal and external care teams to ensure seamless transitions.

    + Educate patients and caregivers to support discharge readiness and long-term care goals.

    + Document care delivery, patient response, and teaching activities in Epic.

    + Refer patients to appropriate community resources, rehab, and long-term care facilities.

    + Address social and psychological needs of patients, including protective services and advance directives.

    + Participate in care conferences and interdisciplinary planning.

    Work Environment

    This position is located in a short-term acute care Level IV Trauma Center with 60 staffed beds and a daily census of approximately 40 patients. The work schedule is Monday to Friday, 7:00 AM to 3:00 PM, with two weekend shifts required per pay period and a potential float requirement for one day per week. Orientation hours are 8. Staff can wear any color scrubs or business casual attire. Free parking is available.

    Pay and Benefits

    The pay range for this position is $40.00 - $55.00/hr.
