    The All-Babies Initiative (ABI, www.allbabies.org) is a program for pregnant women, fathers, and babies up to 18 months old. ABI helps participants find resources in the community, provides education and support to families where most convenient, by offering home visiting services. This position will work hand in hand with healthcare providers and community health workers to ensure UH PNP & Healthy Start compliance by creating and monitoring participant care plans and outreach to ensure social, financial, nutrition and family wellness needs are met.

    Perinatal Navigators Essential Job Duties:

    + Manage a case load of enrolled participants with complex care plans, involving several team members based on identified needs.

    + Travel to client homes, clinics, community agencies, and other outreach destinations to more effectively reduce barriers between clients and institutions

    + Build and maintain positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds.

    + Document and maintain timely, accurate, and thorough records of participant interactions, care coordination and compliance through WFS Case Management system and partner clinic medical record (when applicable).

    + Follow established program guidelines and processes to provide consistent onboarding, support, and evidence-based patient education regarding perinatal topics and best practices throughout pregnancy and until baby is 18 months old.

    + Perform miscellaneous job-related duties as assigned

     

    Education, Training & Experience

     

    + Education: Bachelor’s degree required (family support services, nursing, psychology, social work, or similar)

    + Preferred Skills: Excellent oral and written communication skills, proven critical thinking skills, excellent computer skills, knowledge of multiple computer programs, ability to manage multiple task demands at one time. Spanish proficiency preferred.

    + Physical Demands: Light physical effort: sitting, bending, kneeling, stopping, driving

    + Working Conditions: Ability to work flexible hours

    + Applicant must have a valid driver’s license and working vehicle to successfully facilitate transportation to/from but not limited to: home visits, community visits, clinic sites. (required)
