    Job Descriptions:

    Empowering Mothers, Nurturing Futures! Join Chapa-De Indian Health as a Perinatal Health Representative and make a difference in the lives of mothers, babies, and families in our community.

     

    Hourly Pay: $24.55-28.35 per hour

    Benefit Highlights:

    + Employer-paid Health Insurance (medical, dental and vision): 100% for employee and 80% for dependents.

    + 17 days PTO per year, 11 Paid Holidays, 8+ Sick Days (1 hour for every 30 hours worked)

    + Retirement Plan with 6% match

    Duties:

    + Coordinate and schedule patients for intake session.

    + Develop written individualized care plans and modify as needed.

    + Request and obtain all required medical records from prior care providers for clients referred to the medical department.

    Required Experience:

    Qualifications:

    + One-year prior experience with Electronic Medical Records

    + Provide respectful, patient-centered care while recognizing cultural differences

    + Able to work with little supervision, identify and prioritize tasks
