UPMC Home Health is hiring a full-time Registered Nurse (RN) for the Beaver, Rochester, Freedom and surrounding areas.
Schedule/Shift: Monday - Friday 8am-4:30pm with rotations for on-call, weekends, and holidays.

Responsibilities:
+ Coordinate case management activities within an interdisciplinary healthcare team to ensure timely and appropriate delivery of patient services.
+ Conduct initial and comprehensive patient assessments to inform individualized care planning.
+ Establish and implement patient care plans in collaboration with physicians and other team members, aligning with physician orders and patient needs.
+ Provide direct patient care, monitor outcomes, and adjust nursing interventions to optimize results.
+ Facilitate ongoing communication with physicians, patients, families, and interdisciplinary team members, including health education and support.
+ Assign and supervise appropriate team members, including Licensed practical Nurses (LPNs) and Home Health Aides, to support care delivery.
+ Participate in case conferences, team meetings, and other collaborative forums to support coordinated care.
+ Maintain accurate and timely documentation of assessments, treatment plans, and physician orders using the agency's electronic health record system.
+ Ensure same-day data transmission in compliance with agency and regulatory standards.
+ Attend compliance training and adhere to organizational policies, procedures, and standards of conduct.
+ Demonstrate a commitment to customer service and strive to enhance patient satisfaction through professional and compassionate care.
+ Accept and apply constructive feedback to improve clinical knowledge, skills, and performance.
+ Exercise independent judgement in addressing both emergent and routine patient care needs and escalate concerns appropriately.
+ Identify and contribute to process improvement initiatives that enhance service quality and operational efficiency.
+ Participate in weekend and on-call coverage rotations in accordance with agency guidelines.
+ Exhibit proficiency in delivering care aligned with UPMC's standards, including complex and specialized patient needs.
+ Recognize personal limitations and seek guidance to meet performance goals and uphold quality standards.
+ Zero (0) to two (2) years of experience preferred
+ Six (6) months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care preferred
+ BSN preferredLicensure, Certifications, and Clearances:
+ Registered Nurse (RN) OR Temporary Practice Permit (TPP)
+ Automotive Insurance
+ Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
+ Driver's License

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
