Responsibilities:
GENERAL PURPOSE
With minimal direction and considerable latitude for independent judgement, the referral coordinator is responsible for all aspects of the referrals made to the Telepsychiatry Department from nursing homes across the state of New York. The referral coordinator adheres to approved protocols for working with referrals and makes decisions that are guided by protocols and practices. The referral coordinator maintains an expert level of understanding of the program and division within the department of Psychiatry. The referral coordinator will work closely with the staff in the Telepsychiatry Program, specifically the Health Project Coordinator and Senior Health Project Coordinator. This position serves as a liaison between the telepsychiatry team and our external customers. May train and serve as a resource to new staff. The successful candidate will demonstrate excellent time-management and organizational skills, as well as enthusiasm for supporting our mission to deliver high-quality patient care. Responsible for monitoring own performance on assigned tasks. Self-directed, must make moderate level decisions.
ESSENTIAL FUNCTIONS
Work Queue Referral Coordination
+ Responsible for managing high volume of external referrals to the Telepsychiatry Program.
+ Ensures referrals are complete before scheduling with the nursing home liaison, including contacting referring facility when additional documentation is needed.
+ Sends completed referral documentation for scanning into patient’s medical record.
+ Schedules new patient visits and follow ups with nursing home liaison via zoom invitation, in eRecord as well as the department spreadsheet.
Liaison Responsibilities
+ Handles internal provider as well as referring facility inquiries regarding services.
+ Serves as a point of contact for calls/emails regarding new referrals to the program.
+ Communicates with facility when referral is not complete/ready for scheduling appt.
+ Documents communication on departmental spreadsheet and archives according to department procedures.
+ Assists in problem identification and resolution with Health Project Coordinator and Senior Health Project Coordinator as well as program nurse.
+ Coordinates and communicates efficiently with program leadership for patient/facility issues that arise.
Clinician Support Specialist
+ Serves as master scheduler for all clinical staff. Serves as an eRecord Super User.
+ Assists the customer virtually with Zoom basics and connection issues.
+ Assist providers in the clinic or virtually with same.
eRecord Management
+ Manages multiple processes in eRecord including messaging in eRecord, In Basket and referral work queue processing.
+ Faxes completed documentation to nursing homes via eRecord within one business day of visit.
+ This information is part of the patient legal medical record, therefore, assures accurate and concise information is entered.
Service Coordination
+ Coordination of referrals to other service lines within the department – including teletherapy (Psychology) and Nursing (Nurse Engagement Specialist).
Other duties as assigned.
MINIMUM EDUCATION & EXPERIENCE
+ Associate’s degree in Medical, Secretarial or related field. Preferred
+ High School Diploma and a minimum of two years of relevant experience required
+ Demonstrated customer relations skills. Required
+ Or an equivalent combination of education and experience. Required
+ Medical Terminology, experiences with surgical/appointment scheduling software and electronic medical records, preferred.
KNOWLEDGE, SKILLS AND ABILITIES
+ Excellent organizational, oral and written communication skills; high level of attention to details. Required
+ High level of proficiency with Microsoft Office products including Outlook, Word and Excel. Required
