Full-Time Social Work Associate

Reports To: Director of Social Work

Queens/Manhattan, New York

Responsibilities :

+ Work with clinical social worker to provide social work services at the service units

+ Complete psychosocial assessment for patients as indicated

+ Formulate and implement the intervention plan for patients

+ Provide concrete case management, follow-up and referral service

+ Provide entitlements/benefits eligibility counseling and assist with application

+ Assist Clinical Social Workers in providing case management service as needed

+ Timely services provided to patients based on the Social Work department guideline

+ Complete documentations and reports based on the Social Work department guideline

+ Attend case conference, agency meetings and staff meetings

+ Participate in community outreach and marketing activities

+ Perform other duties as assigned

Minimum Qualifications :

+ Bachelor's Degree in Social Science, Social Work, Psychology, Sociology or other related field

+ At least one year of working experience in social services, health care agency or related field

+ Good verbal and written communication skills

+ Bilingual in English/Chinese (Mandarin/Cantonese) required

Compensation and Benefits :

+ Salary: $50,000.00 - 55,000.00 per year
