Perform assessments of patients in the ER, Community Facilities and on the Hospital Medical Floors to determine eligibility and identify presenting problems to be addressed. Disseminates pertinent findings/data to referral sources as needed. Works with the Program Director to ensure a 24-hour referral call management system has been functioning properly and monitored. Educate staff regularly on referral process. Supports unit staffing needs when necessary.
The working position of Intake Coordinator will assist in daily administrative duties, assist with marketing and community education. May develop and promote educational and public service information programs to the community. Responds to requests for general information about the unit’s treatment program, and provides information about appropriate topics in response to community requests. Provides resources and education surrounding mental health/illness for patients, patient’s families, social agencies and others as needed. Stays thoroughly informed about all clinical services at the hospital and maintains liaison with the hospital’s public relations director on all media and public education or information projects. Serve as a liaison with referral sources to assist in developing and maintaining ongoing working relationships and dialogue regarding the efficacy program services. Identifies mental health/illness needs and interests of the community and referral sources, through relationship building, in the area of mental health and developed strategies to respond in forms of programs, collateral material and/or events. Coordinates admission process from inquiry to admission – working with referral sources, gathering assessment information, and communication with families and staff. Ensure inquiries requesting information about admission, treatment resources in the area, treatment provided by the hospital, or admission criteria and procedures, are responded to in a courteous, professional, and timely fashion and documented on the Inquiry form including electronic and faxed requests and maintain an updated file of local resources for alternative referrals when required. Demonstrates ability to take an inquiry call and perform pre-admission clinical assessments, gather relevant information and communicate findings to the Program Director, Psychiatrist or Medical Staff member to determine appropriateness for admission. Ensure patients are evaluated and placed in the most appropriate level of care. Demonstrates ability to administer a mini-mental status (MMSE)/Saint Louis University Mental Status Exam (SLUMS) tool and any other approved assessment tools. Knowledge of Social Security, Medicare and Medicaid and insurance for behavioral health or experience with legal issues regarding guardianship, consent for psychiatric treatment, DHR and Probate Court Holds, conservatorship, and advance directives/psychiatric advance directives. Understands concepts of excellence in customer service and conducts duties in positive, customer-friendly manner to all customers of the hospital, particularly patients, visitors, physicians, and other employees. Ability to develop and maintain referral and potential referral information in an account management system. Assessing for history of Comorbid Conditions, make certain that the individual’s physical (medical) needs do not take precedence over psychiatric needs at the time that admission is being considered. Attempts to understands the predominant diagnosis of the patient population served and the most common treatment methodologies. Promotes and develops positive, professional interaction with peers, senior management and staff through effective written and verbal communication, listening and feedback in times of organizational change. Census and Referral Development, planning and conducting one-to-one referral source contacts. Review inquiry forms for accurate referral source information, monitor and follow-up on key non-admit inquiries, assures smooth and orderly processes are followed for responding to Intake calls, referrals, and admission into the hospital.
+ Bachelors Degree required in a behavioral health field or Active RN License. Masters in Social Work (MSW), Master’s in Human Service Degree, Masters in Marriage/Family Counseling (MFCC) or Masters in Counseling. A licensed professional such as a Registered Nurse (RN), Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW). BLS by the American Heart Association required. Training available upon hire.
+ + Prefer prior intake, assessment, admission and crisis intervention experience with a psychiatric mental health population, preferably in an inpatient setting. Marketing and community outreach experience preferred
