    Responsibilities

    Lead and direct case management services, workflow and resources.

     

    + Formulate and implement operational strategies and initiatives in the case management program.

    + Direct and evaluate all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives.

    + Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on case management strategies, initiatives, outcomes and issues.

    + Partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care.

    Requirements

    Licensure / Certification / Registration:

    + Required Credential(s):

    + One or more of the following:

    + Case Manager credentialed from the Commission for Case Manager Certification (CCMC) obtained prior to hire date or job transfer date.

    + Counselor obtained prior to hire date or job transfer date.

    + Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date.

    + Social Worker credentialed from the Texas State Board of Social Worker Examiners.

    + Respiratory Care Practitioner obtained prior to hire date or job transfer date.

    + Respiratory Therapist credentialed from the National Board for Respiratory Care (NBRC) obtainedprior to hire date or job transfer date.

    + Social Worker obtained prior to hire date or job transfer date.

    Education:

    + High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required. Master's degree preferred.

    Work Experience:

    + 5 years of experience required.

    + 10 years of experience preferred.

    + 2 years of leadership or management experience required.

    + 5 years of leadership or management experience preferred.

    Additional Preferences

    + Director level Case Management experience

    + Experience working in a large Acute Care hospital setting

    + Energetic, self-motivated professional
