**Shift** : _Days 8am to 5pm_
**Location:** 601 East Rollins Street Orlando, FL 32803 position will require travel to various Central Florida Division facilities

The role you'll contribute:
The Nurse Navigator coordinates all services required to provide multidisciplinary, quality assured, appropriate case selection and cost-effective care to cardiovascular and electrophysiology patients. The Nurse Navigator serves as a resource for patients, physicians and others serving in the role of educator, quality improvement and consultant to patients referred. Makes initial contact with the patient/family to establish a professional relationship and understanding of the diagnosis, treatment and follow-up regimen. The nurse assesses the patient's status and works with physicians to determine appropriate plan for patient and assess appropriateness per regulatory and vendor guidelines. Develops, manages and redesigns, as needed, the process for scheduling a procedure through discharge for patients in the CV department.

Develops, plans, and implements evidence-based measures to improve the quality of care for electrophysiology and cardiovascular patients, ensuring compliance with regulatory guidelines.
Supports and assists with data collection for process and quality improvement, understanding department performance trends and standards.
Navigates patients from the diagnostic phase through treatment, ensuring timely medical and supportive services.
Provides telephone and face-to-face consultations with patients, answering questions, providing educational information, and reviewing discharge information.
Acts as a liaison to physicians' offices regarding proper orders, medical records, consents, and other required documentation for scheduling procedures.
Communicates with patients and families after discharge to address concerns and ensures understanding of follow-up care.
Qualifications
The expertise and experiences you'll need to succeed** **:
EDUCATION AND EXPERIENCE REQUIRED** **:
+ Bachelors degree in Nursing
+ Three years of experience in cardiac care
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED** **:
+ Current valid State of Florida license as a registered nurse
+ Current valid BLS and ACLS certification
